Last week, we hosted our third annual Connecting Care Evening, with generous support from North York General Foundation and Ontario Medical Association. It was an outstanding evening of networking, bringing together more than 170 primary, specialist and community care colleagues as well as local political leaders MP Han Dong and Councillor Lily Cheng to learn more about the ways we can work together towards a more integrated health system. Many thanks to our primary care and specialist colleagues, hospital leadership, OHT partners for attending! #ConnectingCare2024 #PrimaryCare #OntarioHealthTeam #NYTHP North York General Hospital

We were honoured that our Community Health and Information Fairs (#CHIFs) were selected as one of three finalists for the Ontario Health System Quality and Innovation Awards 2024. It was a joy for the members of our dedicated CHIF team who were able to attend the ceremony in-person to accept this recognition and celebrate the incredible work they have accomplished together. We are grateful to be involved in this work and carry this momentum forward in our collective effort in delivering care to our communities. The award celebrates the achievements made by individuals and teams in the Ontario Health system who have gone above any beyond in their service. Many thanks to Dalla Lana School of Public Health, University of Toronto for this recognition, and our partners who help us accomplish this work – North York General Hospital, North York Family Health Team, Working Women Community Centre, VHA Home HealthCare, Flemingdon Health Centre, Unison Health & Community Services, North York Community House, Bernard Betel Centre and Madison Community Services. #OntarioHealthTeam #NYTHP #CommunityCare #IntegratedCare
