For people living in difficult circumstances, the word "free" has a significant meaning - especially when it comes to taking care of yourself. Even more so if one of the most important people in your life needs to receive treatment: your daughter. We are in Lashkar-Gah, #Afghanistan. Little Malalai fractured her arm while playing at home with her siblings. "We rushed to the EMERGENCY hospital because I knew that my little girl would receive the treatment she needs," says her father Wali. "I know this hospital well: a few months ago, I was also treated here after I broke my leg in a car accident." In post-war Afghanistan, among the most urgent health needs are "civilian" trauma injuries – that is, injuries not directly caused by war or conflict. In a country marked by decades of conflict and instability, finding free health facilities that can guarantee sufficient continuity of care and monitoring is increasingly difficult. And for thousands of families, turning to one of the many facilities that require payment is practically impossible: 65% of Afghans live in poverty. Three out of five are children. For Malalai, we have provided free treatment and physiotherapy, helping her recover as quickly as possible. With a schedule of follow-up appointments, we can monitor her health even after discharge. by Francesco Pistilli

Training of national staff is a vital component of EMERGENCY’s capacity building work, which is aimed at strengthening the healthcare systems of the countries where we operate. EMERGENCY is a recognized charity in Belgium, Hong Kong, Italy, Japan, Switzerland, UK, USA. EMERGENCY is a partner of the United Nations (DPI/NGO), and it was granted the Special Consultative Status at ECOSOC in 2015. EMERGENCY has developed the ANME (African Network of Medical Excellence) in order to create Medical Centres of Excellence to strengthen Health Systems at national and international level. In 2015 (Nov. 30th) the founder of EMERGENCY, Dr. Gino Strada, received the "Right Livelihood Award" (Alternative Nobel Prize): “For his great humanity and skill in providing outstanding medical and surgical services to the victims of conflict and injustice, while fearlessly addressing the causes of war”. “The world is already perilously close to the precipice. Every second of delay in reversing course increases the probability of global disaster." Atomic scientists have spoken clearly: we have never been so close to the point of no return. The Doomsday Clock is now 89 seconds to midnight. Wars, nuclear threat, climate crisis, abuse of technologies for increasingly destructive purposes: while the world inches towards the precipice, those who could create change remain inactive. As long as war is treated as an inevitable choice, we will continue to move in the wrong direction.

In our new clinic in #Gaza, we are providing primary care to a population in need. “The population is exhausted after more than 15 months of conflict. The needs created by the poor living conditions will not be resolved in a few days, weeks or months, but will instead take a very long time,” explains Giorgio Monti, EMERGENCY’s Medical Coordinator in Gaza. The clinic is located in the al-Qarara area of Khan Younis. We guarantee first aid, basic medical and surgical care for both adults and children, outpatient reproductive health activities, post-operative nursing follow-up care, stabilisation of medical and surgical emergencies, and transfer to hospitals that are still operational. We also continue to support the work of the primary medical clinic in the ‘humanitarian zone’ in al-Mawasi. Although many families are trying to return to their homes, it is estimated that 69% of all buildings in Gaza have been destroyed. 92% are homes. Despite the start of the ceasefire, needs remain enormous.

We found this shrapnel outside of the Salam Centre for Cardiac Surgery, our hospital in Khartoum, #Sudan. A few days ago, we heard a powerful explosion less than two kilometres away, near Soba Bridge, one of the most important bridges in Khartoum. "The shockwaves from the explosion broke some glass and tiles, and caused cracks in the ceiling. Fortunately, the shrapnel did not hit anyone.” Our hospital is not and has never been a military target, yet this is the second time in the last four months that we have been directly impacted. Fighting continues to intensify in Khartoum, while Sudan experiences an unprecedented humanitarian crisis.

#Gaza. A man of around 25 years old arrives at the clinic. He is very thin, walking with two crutches. A scarf covers his right leg and foot, hiding the surgical rods that have been placed to stabilise the most complex fractures. He has another wound on his left leg, a long surgical scar on his abdomen and an ostomy pouch. There are scars on his face, and his left hand is missing a finger. He weighs just 40 kilos. When we tell him that eating is important for recovery, he laughs: "Eat? Eat what? At the market there is no meat, and an egg and a kilo of flour cost almost $15..." He used to live in Rafah, where he cultivated a small piece of land with his family. Following military operations, he was displaced to Deir Al Balah. One day, his father asked him and his brothers to return to their land for the olive harvest. On their way, they were hit by a grenade launched by a drone. One of his cousins has died. One of his brothers has had a leg amputation. Another brother is still in the hospital, where they are trying to save his right hand. He will have to undergo other surgeries to recover his intestinal function and the use of his leg. He is one of more than 5,000 patients we have seen so far in the field clinic we support in al-Mawasi. In the coastal "humanitarian zone" of the Gaza Strip, two million people are crowded together, exhausted in body and mind, forced to live with nothing
