A 6-month study reveals insights into 𝗰𝗵𝗿𝗼𝗻𝗶𝗰 𝘀𝗽𝗼𝗻𝘁𝗮𝗻𝗲𝗼𝘂𝘀 𝘂𝗿𝘁𝗶𝗰𝗮𝗿𝗶𝗮 (CSU), challenging our understanding of this perplexing condition. Lukas Jörg, 𝗡𝗶𝗰𝗼𝗹𝗲 𝗠𝘂𝗲𝗹𝗹𝗲𝗿-𝗪𝗶𝗿𝘁𝗵, Kevin Kammermann, Odile Stalder, Werner Pichler and 𝗢𝗹𝗶𝘃𝗲𝗿 𝗛𝗮𝘂𝘀𝗺𝗮𝗻𝗻 found a paradoxical relationship between #autoantibody quantity and quality: as CSU patients' autoantibody levels surged, their functional affinity unexpectedly declined. Surprisingly, #basophil activation remained stable despite fluctuating autoantibody levels. This suggests the CU-BAT test might be superior for diagnosing #autoimmune CSU compared to antibody measurements alone. This research not only challenges assumptions but paves the way for a more nuanced understanding of CSU. It's a must-read for anyone looking to scratch beneath the surface of this complex condition!

Eosinophilic esophagitis (EoE) is a chronic 𝗲𝘀𝗼𝗽𝗵𝗮𝗴𝗲𝗮𝗹 disease linked to atopy. The study of Leonor Esteves Caldeira, Rita Limão, Rita Brás, Elisa Pedro and Célia Costa aimed to see if sensitization to airborne and food #allergens correlates with EoE severity and to evaluate clinical and laboratory characteristics. The association between age at diagnosis, disease duration, allergen sensitization, serum IgE, blood #eosinophil counts, and severe clinical symptoms impacting quality of life and and histological #disease was evaluated. Among 92 patients (83% male, 87% atopic), the average diagnosis delay was 4 years. #Sensitization to #aeroallergens was 84%, and food sensitization was 71%. Severe clinical disease was seen in 55%, and histological severity in 37%. Severe clinical disease was linked to longer disease duration before diagnosis. 𝗣𝗮𝘁𝗶𝗲𝗻𝘁𝘀 𝘄𝗶𝘁𝗵 𝗳𝗼𝗼𝗱 𝗶𝗺𝗽𝗮𝗰𝘁𝗶𝗼𝗻 𝘄𝗲𝗿𝗲 𝗼𝗹𝗱𝗲𝗿 𝗮𝘁 𝗱𝗶𝗮𝗴𝗻𝗼𝘀𝗶𝘀. No significant link was found between allergen sensitization, serum IgE, blood eosinophils, and disease severity.

Rosella Gallo, Ilaria Trave, 𝗚𝗶𝘂𝗹𝗶𝗮 𝗚𝗮𝘀𝗽𝗮𝗿𝗶𝗻𝗶, 𝗥𝗶𝗰𝗰𝗮𝗿𝗱𝗼 𝗖𝗮𝘀𝘁𝗲𝗹𝗹𝗶, 𝗚𝗶𝘂𝗹𝗶𝗮 𝗠𝗲𝗿𝗹𝗼, aurora parodi, 𝗘𝗺𝗮𝗻𝘂𝗲𝗹𝗲 𝗖𝗼𝘇𝘇𝗮𝗻𝗶 discuss a rare form of chronic inducible urticaria (CIndU) known as 𝗮𝗾𝘂𝗮𝗴𝗲𝗻𝗶𝗰 𝘂𝗿𝘁𝗶𝗰𝗮𝗿𝗶𝗮 (AquaU), specifically focusing on a 𝘀𝗮𝗹𝘁-𝗱𝗲𝗽𝗲𝗻𝗱𝗲𝗻𝘁 𝘀𝘂𝗯𝘁𝘆𝗽𝗲 (SDAU). AquaU is a rare condition where contact with #water, regardless of temperature, causes #hives on the #skin. Predominantly affects young #females; most cases are sporadic. The exact cause is unknown, but hypotheses include the generation of a 𝘁𝗼𝘅𝗶𝗰 𝘀𝘂𝗯𝘀𝘁𝗮𝗻𝗰𝗲 𝗼𝗿 𝗮𝘂𝘁𝗼-𝗮𝗹𝗹𝗲𝗿𝗴𝗲𝗻 𝗳𝗿𝗼𝗺 𝘄𝗮𝘁𝗲𝗿-𝘀𝗼𝗹𝘂𝗯𝗹𝗲 𝗰𝗼𝗺𝗽𝗼𝗻𝗲𝗻𝘁𝘀 𝗶𝗻 𝘁𝗵𝗲 𝘀𝗸𝗶𝗻. The symptoms are highly pruritic urticarial #rashes triggered by #sea bathing, tap water, #swimmingpool water, and #sweat. Nine female patients diagnosed with SDAU underwent tests with tap water and a 3.5% NaCl solution. The tests helped identify the #𝘀𝗲𝗻𝘀𝗶𝘁𝗶𝘃𝗶𝘁𝘆 𝘁𝗼 𝘀𝗮𝗹𝘁𝘆 𝘄𝗮𝘁𝗲𝗿, suggesting that #salinity might be a relevant parameter for AquaU.

A 18-month-old male without a history of atopic #diseases developed a pruritic rash 𝟮𝟬 𝗺𝗶𝗻𝘂𝘁𝗲𝘀 after consuming #oatmeal with #flaxseed. 𝗦𝗸𝗶𝗻 𝗽𝗿𝗶𝗰𝗸 𝘁𝗲𝘀𝘁𝘀 revealed #sensitization to various #nuts and #seeds, including flaxseed. It is hypothesized that the child was 𝗰𝘂𝘁𝗮𝗻𝗲𝗼𝘂𝘀𝗹𝘆 𝘀𝗲𝗻𝘀𝗶𝘁𝗶𝘇𝗲𝗱 𝘁𝗵𝗿𝗼𝘂𝗴𝗵 𝗵𝗶𝘀 𝗺𝗼𝘁𝗵𝗲𝗿, who consumed flaxseed during #pregnancy and #lactation. 𝗔𝘃𝗼𝗶𝗱𝗮𝗻𝗰𝗲 of flaxseed 𝗶𝘀 𝗿𝗲𝗰𝗼𝗺𝗺𝗲𝗻𝗱𝗲𝗱, but the child can consume products with minimal traces. 𝗔𝗹𝗮𝗻𝗮 𝗫𝗮𝘃𝗶𝗲𝗿 𝗱𝗲 𝗔𝗹𝗺𝗲𝗶𝗱𝗮, Travis Satnarine, 𝗝𝗲𝗻𝗻𝗶𝗳𝗲𝗿 𝗚𝗲𝗯𝗯𝗶𝗮, Hugh. The sudy signed by Pedro Simão Coelho, Gonçalo Martins dos Santos, 𝗠𝗶𝗹𝗮 𝗠𝗶𝗸𝗼𝘃𝗶𝗰, 𝗝𝗼ã𝗼 𝗢𝗹𝗶𝘃𝗲𝗶𝗿𝗮, 𝗦ó𝗻𝗶𝗮 𝗥𝗼𝘀𝗮, 𝗘𝗱𝘂𝗮𝗿𝗱𝗼 𝗦𝗶𝗹𝘃𝗮 and Paula Leiria Pinto evaluates the #frequency of #drug allergies and their impact on treatment among hospitalized patients in a Lisbon hospital. It involved a retrospective analysis of 3,046 adult patients with documented drug allergies from 2018 to 2020. The study found that 𝟭𝟱.𝟰% 𝗼𝗳 𝗽𝗮𝘁𝗶𝗲𝗻𝘁𝘀 𝗵𝗮𝗱 𝗮 𝗱𝗿𝘂𝗴 𝗮𝗹𝗹𝗲𝗿𝗴𝘆 𝗹𝗮𝗯𝗲𝗹, 𝗵𝗶𝗴𝗵𝗲𝗿 𝘁𝗵𝗮𝗻 𝘁𝗵𝗲 𝗴𝗲𝗻𝗲𝗿𝗮𝗹 𝗽𝗼𝗽𝘂𝗹𝗮𝘁𝗶𝗼𝗻'𝘀 𝟴%. The most commonly reported allergens were #antibiotics, non-steroidal #antiinflammatory drugs, and #radiocontrast media. Drug allergy labels led to the use of second-line treatments in 14.5% of cases, 𝗶𝗻𝗰𝗿𝗲𝗮𝘀𝗶𝗻𝗴 𝘁𝗿𝗲𝗮𝘁𝗺𝗲𝗻𝘁 𝗰𝗼𝘀𝘁𝘀 𝗯𝘆 𝟮.𝟰 𝘁𝗶𝗺𝗲𝘀. However, many allergy records were incomplete, and some had previously tolerated the drug without adverse reactions, suggesting potential #overdiagnosis. Only 1.9% of patients were referred for allergy evaluation post-discharge, with several allergy suspicions later excluded after proper testing. The study suggests that 𝗼𝘃𝗲𝗿𝗱𝗶𝗮𝗴𝗻𝗼𝘀𝗶𝘀 𝗼𝗳 𝗱𝗿𝘂𝗴 𝗮𝗹𝗹𝗲𝗿𝗴𝗶𝗲𝘀, especially in hospitalized patients, 𝗹𝗲𝗮𝗱𝘀 𝘁𝗼 𝘂𝗻𝗻𝗲𝗰𝗲𝘀𝘀𝗮𝗿𝘆 𝗮𝘃𝗼𝗶𝗱𝗮𝗻𝗰𝗲 𝗼𝗳 𝗰𝗲𝗿𝘁𝗮𝗶𝗻 𝗱𝗿𝘂𝗴𝘀, resulting in 𝗵𝗶𝗴𝗵𝗲𝗿 #costs and potential treatment complications. Improved #communication between departments, thorough documentation, and routine allergy evaluation, are essential to enhance patient care and reduce #healthcare costs associated with drug allergy labels.
