Today, we have issued a safety bulletin on the investigation of a fatal man overboard from the potter Kingfisher (DH110), approximately 30 nautical miles east-north-east of Wick, Scotland on 12 July 2024. * Make sure your risk assessments identify PPE snagging hazards. * Use risk assessments to identify what PPE is required to be worn for different tasks. * Purchase appropriate PPE for the work involved, taking into account detailed risk assessments. * Communicate to and involve all crew members on the suitability of PPE.

Today, we have published our accident investigation report into the flooding and partial sinking of an inflatable migrant boat resulting in the loss of at least 8 lives in the Dover Strait. * Only use internationally recognised methods to alert the authorities that you are in distress. Emails, tweets or other informal methods might be missed by the appropriate authorities.

Today, we have published our accident investigation report into the capsize and foundering of the fishing vessel Angelena (BM271) south-east of Exmouth, England on 18 June 2021. * Angelena capsized when its stability was compromised on lifting a heavy load. * Sailing with little fuel on board can reduce stability. Do a roll or heel test for your vessel. * Liferafts must be able to automatically float free, inflate and break free. * The skipper was wearing a PFD and lifesaving equipment worked well.

Today, we have published our accident investigation report into the investigation of the grounding of swim support motor vessel Channel Queen, near the Needles Lighthouse, Isle of Wight, England on 20 July 2023. * Plan your passage – ineffective passage plans can lead to groundings or other similar accidents. * Prioritise safe navigation by a competent crew to ensure successful task outcomes. * Stay focused. It is essential that those tasked with navigating the vessel are undistracted by other work.

Today, we have published our accident investigation report into the contact, by Ali Ka, with the westernmost section of Oikos Jetty 2 at Canvey Island on the River Thames, England on 25 October 2022. * This serious accident highlighted the potential risks of a very serious accident occurring. * Fatigue needs active management to control the associated risks. * Challenges to prevent this accident went unheeded.
