Californians have continued to lose access to labor and delivery services. This year, four hospitals have shut down their maternity wards, with another four slated to close by November. That’s nearly on par with the 10 maternity ward closures in 2023. Hospital administrators blame service cuts on high costs, labor shortages and declining birth rates. But in some instances, CalMatters found highly profitable hospitals closing maternity wards that delivered more than 700 babies annually, a number that research suggests is well above the threshold for safety and financial viability. The trend has drawn attention from lawmakers and health advocates who have asked Gov. Gavin Newsom to sign legislation intended to help the state intervene when a hospital is struggling to keep labor and delivery open. They also want to force hospitals to be more transparent about their finances, #staffing levels and how closures would impact communities. Innovative solutions can reduce hospitals need for closures. This convenience ensures more consistent prenatal check-ups and timely interventions. 2️⃣ 𝗖𝗼𝗻𝘁𝗶𝗻𝘂𝗶𝘁𝘆 𝗼𝗳 𝗖𝗮𝗿𝗲: Through telehealth platforms, healthcare providers can maintain regular communication, monitor vital signs, and adjust treatment plans based on real-time data, enhancing the quality of care and ensuring prompt responses to emerging complications. 3️⃣ 𝗣𝗮𝘁𝗶𝗲𝗻𝘁 𝗘𝗺𝗽𝗼𝘄𝗲𝗿𝗺𝗲𝗻𝘁: Virtual care fosters greater patient engagement by allowing women to take an active role in managing their health. Access to educational resources and the ability to ask questions in real-time contribute to informed decision-making and better health outcomes. 4️⃣ 𝗖𝗼𝘀𝘁-𝗘𝗳𝗳𝗲𝗰𝘁𝗶𝘃𝗲𝗻𝗲𝘀𝘀: By reducing the need for physical visits, telehealth cuts down travel expenses and time off work, making maternal healthcare more affordable and accessible. 5️⃣ 𝗦𝗮𝗳𝗲𝘁𝘆 𝗔𝗺𝗶𝗱 𝗣𝗮𝗻𝗱𝗲𝗺𝗶𝗰𝘀: The COVID-19 pandemic highlighted the importance of minimizing in-person visits to reduce the risk of exposure. With over one in eight individuals experiencing disrespectful and harmful treatment, particularly those in marginalized groups, the findings are alarming. 🔸 Common forms of mistreatment include being ignored, shouted at, or coerced into unwanted treatment, indicating deep-rooted structural stigmas. This mistreatment not only undermines dignity but also leads to long-term psychological impacts and exacerbates maternal health disparities. 🔸 These insights highlight an urgent need for reforms that ensure respectful maternity care to safeguard the well-being of all birthing individuals. 💡 𝗧𝗼𝗽 𝗥𝗲𝗮𝘀𝗼𝗻𝘀 𝗳𝗼𝗿 𝘂𝘀𝗶𝗻𝗴 𝗱𝗶𝗴𝗶𝘁𝗮𝗹 𝗵𝗲𝗮𝗹𝘁𝗵 𝗿𝗲𝘀𝗼𝘂𝗿𝗰𝗲𝘀 𝗱𝘂𝗿𝗶𝗻𝗴 𝗽𝗿𝗲𝗴𝗻𝗮𝗻𝗰𝘆 𝗮𝗻𝗱 𝘁𝗵𝗲 𝗽𝗼𝘀𝘁𝗽𝗮𝗿𝘁𝘂𝗺 𝗽𝗲𝗿𝗶𝗼𝗱: Digital health is widely used by 90%-97% of pregnant individuals, highlighting its growing role in the perinatal period. In a recent research conducted by Natalie Henrich et al., results show: ➡️ 𝗧𝗼𝗽 𝗿𝗲𝗮𝘀𝗼𝗻𝘀 𝗳𝗼𝗿 𝘂𝘀𝗶𝗻𝗴 𝗱𝗶𝗴𝗶𝘁𝗮𝗹 𝗵𝗲𝗮𝗹𝘁𝗵 𝗿𝗲𝘀𝗼𝘂𝗿𝗰𝗲𝘀 𝗱𝘂𝗿𝗶𝗻𝗴 𝗽𝗿𝗲𝗴𝗻𝗮𝗻𝗰𝘆 were to (1) know what is normal or typical during pregnancy, (2) have access to a health care provider when needed, and (3) know how the baby is developing. Top reasons for postpartum use were to (1) help with breastfeeding, (2) know what normal baby development is, and (3) help with the baby’s health issues. ➡️ 𝗧𝗼𝗽 𝗽𝗹𝗮𝘁𝗳𝗼𝗿𝗺 𝗳𝗲𝗮𝘁𝘂𝗿𝗲𝘀 𝗱𝘂𝗿𝗶𝗻𝗴 𝗽𝗿𝗲𝗴𝗻𝗮𝗻𝗰𝘆 𝗮𝗻𝗱 𝘁𝗵𝗲 𝗽𝗼𝘀𝘁𝗽𝗮𝗿𝘁𝘂𝗺 𝗽𝗲𝗿𝗶𝗼𝗱 were (1) credible and trustworthy information and providers, (2) nonjudgmental information and support, and (3) no cost to the user. 🇺🇸 𝗠𝗮𝘁𝗲𝗿𝗻𝗮𝗹 𝗛𝗲𝗮𝗹𝘁𝗵 𝗖𝗿𝗶𝘀𝗶𝘀: 𝗠𝗮𝘁𝗲𝗿𝗻𝗮𝗹 𝗠𝗼𝗿𝘁𝗮𝗹𝗶𝘁𝘆 The US maternal health record is worse than any peer country in the world. The outcomes and disparities seen in maternal health make clear that our current approach to prenatal care, the backbone of prevention in pregnancy, is inadequate for addressing the complex factors that people face as they navigate their pregnancy. Digitally enabled prenatal care, which includes multiple services designed to meet the individual’s needs within and outside of clinical spaces, can improve maternal health and wellness in the US by impacting parts of people’s lives that are beyond current clinical reach. Despite the alarming increase in the US maternal mortality rate, which is two to three times higher than other high-income countries, mental health disorders such as suicide and opioid overdose account for nearly a quarter of maternal deaths. 𝗧𝗵𝗲𝘀𝗲 𝗱𝗲𝗮𝘁𝗵𝘀 𝗮𝗿𝗲 𝗹𝗮𝗿𝗴𝗲𝗹𝘆 𝗽𝗿𝗲𝘃𝗲𝗻𝘁𝗮𝗯𝗹𝗲, yet there is insufficient emphasis on mental health in maternal healthcare initiatives, and significant gaps remain in screening, access to care, and public and policy attention. Additionally, systemic inequities exacerbate these issues, particularly for racial and ethnic minority groups. 𝗥𝗲𝗰𝗼𝗺𝗺𝗲𝗻𝗱𝗮𝘁𝗶𝗼𝗻𝘀 𝘁𝗼 𝗮𝗱𝗱𝗿𝗲𝘀𝘀 𝗺𝗮𝘁𝗲𝗿𝗻𝗮𝗹 𝗺𝗲𝗻𝘁𝗮𝗹 𝗶𝗹𝗹𝗻𝗲𝘀𝘀 𝗮𝗻𝗱 𝗶𝗺𝗽𝗿𝗼𝘃𝗲 𝗺𝗮𝘁𝗲𝗿𝗻𝗮𝗹 𝗵𝗲𝗮𝗹𝘁𝗵 𝗼𝘂𝘁𝗰𝗼𝗺𝗲𝘀 𝗶𝗻 𝘁𝗵𝗲 𝗨𝗦 𝗶𝗻𝗰𝗹𝘂𝗱𝗲: Improving interdisciplinary training; promote public education; investing in community services; reducing systemic barriers such as transportation; and enhancing access to comprehensive maternity care in "maternity care deserts". ⚠️ 𝗠𝗮𝘁𝗲𝗿𝗻𝗮𝗹 𝗵𝗲𝗮𝗹𝘁𝗵 𝗰𝗿𝗶𝘀𝗶𝘀: 𝗠𝗮𝘁𝗲𝗿𝗻𝗶𝘁𝘆 𝘄𝗮𝗿𝗱𝘀 𝗮𝗿𝗲 𝗰𝗹𝗼𝘀𝗶𝗻𝗴 𝗿𝗮𝗽𝗶𝗱𝗹𝘆 𝗮𝗰𝗿𝗼𝘀𝘀 𝗖𝗮𝗹𝗶𝗳𝗼𝗿𝗻𝗶𝗮 Maternity wards are closing rapidly across California. This year, 11 hospitals shut down their labor and delivery departments. Since 2012, 46 hospitals have discontinued these services, with 27 closures in the last three years. These closures hit low-income and Latino communities hardest. In Imperial County, only one hospital remains to deliver about 2,500 babies annually, forcing some women to travel nearly two hours for care, leading to delays and strained facilities. Amid a broader maternal health crisis, financial stress, high operational costs, labor shortages, and a declining birth rate are driving these closures. This crisis requires alternative solutions.
