CMS has extended the ACO Primary Care Flex Model application deadline to Friday, 8/23 at 11:59P ET (previously 8/1). Note: this does not impact existing Shared Savings Program application actions and deadlines. Visit the ACO PC Flex Model application portal to apply: go.cms.gov/pcflex.
