Yesterday, Cityblock's Toyin Ajayi & Destiny Fernandez joined New York Governor Kathy Hochul for the announcement of the state’s investment in SBH Health System. The Bronx community that Cityblock serves through our collaboration with St. Barnabas Hospital will benefit greatly from the state’s investment in both emergency department care as well as critical behavioral health and care management services. Today, we published our third annual report, "Spotlight on the Dual Eligible Landscape", which highlights Cityblock’s integrated care model and our ability to build strong, trusting relationships with members who are navigating the complexities of both Medicare and Medicaid benefits.   The dually eligible population is continuously growing and is estimated to reach 15 million beneficiaries by 2030. This report highlights the intricacies of navigating two separate health plans, how policy is shifting to help simplify the complexities for dually eligible members, and Cityblock’s approach to engaging this traditionally hard-to-reach population. Today, a new peer-reviewed article that was published in NEJM Catalyst shows Cityblock's Advanced Behavioral Health Program led to a significant decrease in inpatient utilization and healthcare costs for Medicaid members with severe mental health conditions. Here are some key takeaways: ✅ Value-based contracts that integrate medical, behavioral, substance use disorder and social care driven by community health workers, can enable effective integrated services and meet public health needs, for populations on publicly funded health plans. ✅ Our particular community health worker-led, integrated model of care can improve access to evidence-based care for people with psychotic disorders and substance use disorders, including long-acting injectable antipsychotics, medications for opioid and alcohol use disorders, and social care navigation. ✅  This examination of Cityblock’s ABH program shows a statistically significant reduction in inpatient hospitalizations and total cost of care and makes the case for further investment in public health.
