    Care in the home is provided under such varied business models...from franchised family funded personal care, to Medicare certified skilled home health, pediatrics, in-home primary care, hospice care, rising acuity care being delivered at home, and serving diverse Medicaid populations both urban and rural.     Re-imagining the relationship between healthcare employers and caregivers in the new economy

    Observation #6 from my first Home Care 100: The Tesseract of HH At one of the reception dinners at HC100, an artist displayed an incredible work called "The Tesseract". I wondered if it was an accident, or an active choice made by the organizers. You see in geometry, a tesseract is a "4-dimensional hypercube"- a cube, if it had a 4th dimension. Metaphorically, it refers to how complex and multi-dimensional most things are. It made me think about and appreciate the multi-dimensional tesseract a HH provider must keep in their heads, at a time of incredible change. Must feel something like this... 1. Opportunity Everywhere: HH is growing at an astounding 7.96% CAGR. What a great tailwind to ride, but.... 2. Caregiver Shortage: Caregivers are my product, and product remains in short supply. I need to invest more in my people, except... 3. Economic Pressure: Payment risk is existential. Interest rates are high. Multiples are compressed, and strategics are looking differently at the quality of my business. I have to tighten the screws on my business, just to run in place. But also... 4. Looming 80/20: I'll need to find ways to do more with less, and so... 5. The Rise of AI: I need to start thinking about re-imagining my cost structure with technology to improve our unit economics, but also.... The dimensions must feel near-infinite. Leaders from across home-based care, including CEOs from the largest operators in the country, policy insiders, private equity, payers and leading-edge product/service companies came together at Home Care 100 last week to connect across diverse viewpoints on key issues impacting the future of the sector and develop critical partnerships.     Re-imagining the relationship between healthcare employers and caregivers in the new economy

    Day #3 of observations from attending Home Care 100 last week: I attended a roundtable of about 30 Home Health executives about Artificial Intelligence/ Automation, and took full notes of their comments. I then ran these notes through a semantic algorithm to categorize them based on their current mindset around AI and automation. Here are the results: 40%: "✋🏼 Wait and watch". Skeptical, cautious or feeling generally unprepared about diving into AI without sufficient understanding or observable benefits. 45%: " 🤗 I want to be at the leading edge". Interested in implementing AI/ automation, but within a structured, conservative framework. 15%: " 😍 I want to be at the bleeding edge". Risk-takers who want to experiment with AI to its fullest potential, irrespective of the broader adoption trends. Most executives want to be at the leading edge, not the bleeding edge, on AI and automation. Cautious, as we should be in healthcare. We can debate about what the "bleeding edge" might be- it's probably putting AI somewhere between the clinician and the patient. But what should be the "leading edge"? Scott Powers from Elara framed this up perfectly: "30% of the margin of a clinical business goes through to SG&A" I found this number to be high, and I pressure tested it with a few executives at the conference who agreed. And so I pulled out my nerd goggles again 🤓 . So I built a waterfall of a well-run mid-market home health business and confirmed this to be the case. You talk to the senior executive of this business, and they believe there is a lot of bloat from manual scheduling, care coordination and onboarding and credentialing workflows that should flow to the bottom line. Supercharging the productivity of folks in your SG&A line to be able to handle 2x the revenue. That's the leading edge. #homecare #homecareagencies #HC100 #homecare100 #homehealth
