Our Federally Qualified Community Health Center serves more than 66,000 individuals of all ages in Alameda, Contra Costa, and Marin Counties each year with medical, dental and/or social services.     I am proud of our commitment to community health at LifeLong Medical Care, which extends far beyond the walls of our clinics. LifeLong recently attended two events that exemplify our dedication to serving diverse communities and addressing critical issues. First, LifeLong sponsored the 38th Annual Greater Richmond Interfaith Program (GRIP) Harmony Walk. This crucial fundraiser combats hunger and homelessness in Richmond, bringing together live music, food vendors, and local businesses. For me, it's more than just an event – it's a powerful demonstration of how unity can address the challenges faced by our unsheltered neighbors. Just a week earlier, our team participated in Berkeley Unified School District’s Latino Heritage Month celebration. We provided health services and information, joining families, students, and community members for an afternoon of culture, music, and wellness. This event truly embodies our commitment to inclusivity and ensuring access to resources for our Latinx community. These initiatives reinforce what I believe LifeLong stands for: - Addressing health disparities - Fostering community connections, and - Supporting underserved populations across the Bay Area. I see us not just as healthcare providers but as community partners 🤝. And I'm excited about what's to come.     October is American National Pharmacists Month, a month dedicated to highlighting the crucial role pharmacists play in our health and well-being. This month is especially significant for Kyle Hui, PharmD BCGP, LifeLong’s Pharmacy Director, and only pharmacist. Pharmacists like Hui are essential to healthcare organizations and play a critical role in ensuring optimal patient care, medication safety, and improved health outcomes. Pharmacists work alongside other healthcare professionals to provide patient-centered, team-based care, which includes counseling patients on medication use, managing medication therapy, and addressing medication-related problems. Their expertise helps expand access to care, improve medication adherence, and reduce healthcare costs. The patient-centered care is what attracted Hui to LifeLong. “I felt like I wanted to use the knowledge that I have to help the providers here and the patients to get what they need,” he said. As LifeLong’s only pharmacist, Hui’s responsibilities are extensive and vital. He not only orders medications for the dispensaries but also oversees the vaccine program and acts as the liaison between LifeLong and local pharmacies. Additionally, Hui and his staff ensure LifeLong’s compliance with the intricate federal 340B program, which mandates drug manufacturers provide discounts to low-income and uninsured patients. #AmericanPharmacistsMonth #ThankAPharmacist #PharmacyMonth #PharmacyHeroes

    🎉Congratulations to the class of 2024 Nurse Practitioners graduates! Seven nurse practitioners from the East Bay Consortium’s Advanced Practice Provider (APP) Fellowship graduated in September. The consortium is a partnership with LifeLong, La Clinica and UCSF-School of Nursing. The thrilling news is that six will continue working at LifeLong, while the seventh graduate has signed on with La Clinica. 🩺 “At the highest levels of leadership at LifeLong, we are so proud of this program and these graduates,” said Chief Medical Officer Dr. Michael Stacey. “This is really fulfilling our intention of training advanced providers, not just in community health, but to stay on and work at LifeLong.” 🤝The APP Fellowship rigorous 12-month program is an integral part of LifeLong’s commitment as an education institution dedicated to training providers in community health. This program not only helps address the shortage of nurse practitioners and physician assistants in the medical community in general, but it also provides an important pipeline to filling critical positions at LifeLong. 🎉Congratulations to the new graduates!     💙LifeLong honored the life and legacy of Dr. Howard Daniel, whose East Oakland practice became a part of LifeLong Medical Care 16 years ago, was celebrated last month by local dignitaries, former patients, and his extensive family. Dr. Daniel passed away on June 15, 2022, days before his 89th birthday. “His practice, started in 1968, was not just a medical office but a sanctuary for so many in this community,” Alameda County Board President Nathan Miley said. “He faced and overcame systemic barriers, starting with his own practice, when Black doctors were often excluded. This was not just an act of personal triumph, but an act of resistance against a system that continues to leave too many behind. In honoring him today, we recognize the critical need to continue dismantling these barriers today, as we face the challenge of ensuring access to comprehensive healthcare for all.” Miley continued, “Dr. Daniel’s work aligned with our ongoing commitment to health equity, and it reminds us that investment in community-based healthcare can change lives.” Many who spoke noted that Dr. Daniel, raised in Ohio and Jamaica, Queens, New York, and educated at Howard University College of Medicine, could have practiced anywhere, but he chose East Oakland and kept his practice in the community for 45 years. During that time, he delivered close to 5,000 babies, his son Kent Daniel told the gathering. He merged his practice with LifeLong in 2008, when it became LifeLong’s Howard Daniel Health Center.     🎗️October is Breast Cancer Awareness Month, a powerful reminder of the importance of early detection. We are proud of the work we’re doing to help women in our community get the screenings they need. In partnership with Sutter Health, we’ve been able to bring mammography-equipped vans directly to underserved neighborhoods, breaking down the barriers that too often prevent women from accessing breast cancer screenings. Breast cancer screenings save lives, if you’re due for a mammogram make an appointment. According to American Cancer Society here some screening guidelines: 🎗️Women between 40 and 44 have the option to start screening with a mammogram every year. 🎗️Women 45 to 54 should get mammograms every year. 🎗️Women 55 and older can switch to a mammogram every other year, or they can choose to continue yearly mammograms. 🎗️Screening should continue as long as a woman is in good health and is expected to live at least 10 more years. 🎗️All women should understand what to expect when getting a mammogram for breast cancer screening – what the test can and cannot do. ➡️Make an appointment for your mammogram today by calling 510-981-4000.

Last Thursday, LifeLong Medical Care’s board, staff, and community health colleagues gathered to celebrate the remarkable career of our CEO Emeritus, David B. Vliet, MBA. 🎉 The event was filled with stories, laughter, and love as we reminisced about David's compassionate, visionary, and steadfast leadership, his great sense of humor, and his impressive musical talents. 💖 We were honored to have remarkable leaders join us, including Andie Martinez Patterson (CEO, Alameda Health Consortium and Community Health Center Network), Tracy Macdonald Mendez (CEO, Aliados Health), and Jane Garcia (CEO, La Clínica). Their kind words and commitment to community health made the event even more special✨. 📢Special shout-outs to RYSE Youth Center and Blackberry Soul Fine Catering. 🤝Over many years, no matter the issues or challenges, David put equity for patients and the workforce of community health centers at the center, transforming countless lives.     Reflecting with gratitude this past week, Gracy Trinoskey-Rice and I had the privilege of visiting 13 community health centers (CHCs) in Northern California with the great staff of the California Primary Care Association (CPCA) and 3 regional primary care associations (PCAs): Alameda Health Consortium, Community Health Partnership, and San Francisco Community Clinic Consortium (SFCCC). We were so impressed with their diversity, innovation, impact, and the power of UNITY in commUNITY. During 1976 in Berkeley, California, the senior population faced a growing, unmet need for health services. Enter a small group of Gray Panthers, whose members created an innovative solution—a milestone in elder care—the Over 60 Health Center. The first health center was located in a storefront on San Pablo Avenue in Berkeley has evolved to become LifeLong Medical Care, a dynamic nonprofit community organization. They provide medical, dental, behavioral health and social services in Alameda and Contra Costa Counties. Each year, they deliver comprehensive primary care and health equity for 50K+ patients each year, many with complex health conditions. They serve all, regardless of ability to pay, housing situation or immigration status. They do this through 14 primary health centers, several of which also offer urgent and dental care. In addition, they are out in the community with 2 mobile dental centers and 5 street medicine vans. They also provide supportive housing, medical respite, and school-based health services.
