Advanced care planning (ACP) is crucial for seniors and healthcare professionals. The National Institute on Aging emphasizes ACP as a way to ensure medical care aligns with patients' wishes, especially in critical situations where they cannot communicate. ACP involves making decisions about the care you would want to receive if you become unable to speak for yourself, or your loved ones. This includes discussing and documenting treatment preferences. Start conversations early and revisit them periodically. 🌟 Exciting News! Troupe Health Joins CMS Innovation GUIDE Model Program 🌟 We are thrilled to formally announce that Troupe Health has been selected by the Centers for Medicare & Medicaid Services (CMS) to participate in the groundbreaking Guiding an Improved Dementia Experience (GUIDE) Model program! 🎉 Under this innovative program, Troupe Health will be one of nearly 400 participants nationwide working to build Dementia Care Programs (DCPs) aimed at enhancing care coordination and providing critical support to individuals living with dementia and their caregivers. Our goal is to improve access to essential services and support systems, including respite care, enabling individuals to age in place within their communities. The GUIDE Model, which Troupe Health will start on 7/1/2025, introduces a new payment approach for comprehensive, person-centered assessments, care coordination, and 24/7 access to interdisciplinary care teams. It also offers invaluable resources such as meals, transportation, and caregiver training programs through community-based organizations. Steve Weissblum, CEO of Troupe Health, shared his enthusiasm: "We're excited to be part of this innovative CMS program to better support residents of our partner senior living communities with dementia and their caregivers. The GUIDE model will offer much-needed support and resources to help our residents age in place."
