March is National Social Worker Month, and this year's theme is Compassion + Action! We are proud to celebrate the compassion and resourcefulness of social workers on VHAN's Care Management team. VHAN Social Worker Cate Mart, LMSW, CCM and her team regularly engage patients after a hospital stay to ensure the patient has everything they need to manage their care. This case-based educational program is aimed at strengthening clinical knowledge of cardiovascular and neurologic care for female patients across their life span. Expert faculty from VUMC will discuss key issues in the care of both reproductive age and post-menopausal patients, as well as provide breakout sessions for cardiac and neurological topics.
