The proliferation of GLP-1s has taught us a lot about consumers — they'll find a way to get on a given drug if they want it badly enough. PBMs help avoid waste through population-level management, but to best eliminate the skyrocketing costs, we need to implement active medication management for the individual. In honor of the 35th anniversary of one of my favorite movies, I hope you'll consider the following question: What does "Field of Dreams" have to do with pharmacy benefits? When the film was released on April 21, 1989, the line "If you build it, he will come" was forever etched in our memories. It's another classic line, however, that's relevant for how employers can address wasteful spending on prescription drugs: "Go the distance." When Kevin Costner's character hears "The Voice" say this at the Red Sox game, he's convinced to see his plan through to the end. Similarly, in order for employers to root out and address Rx waste among their costliest plan members taking five or more medications, they too need to "go the distance" and extend medication management programs from the population level all the way down to the individual level. Just because someone *can* be on a given medication doesn't mean they *should* be. That's the gist of my friend Kimball Thomas's company, Walrus Health, which takes familiar principles that PBMs use at the population level to actively manage medications at the individual level, where a surprising amount of waste (and dangerous issues) slips through things like formulary, prior-auth, etc. I'm moderating a webinar on the topic for them on Wednesday.
