The clinical efficacy data on various forms of PAT—psychedelic-assisted therapy—looks promising as an alternative for treatment-resistant mental health conditions, especially where traditional interventions have failed. As such, it's crucial to make these innovative treatments accessible to all who need them. Affordability through insurance coverage is key to that equitable access, as it’s common that the patients that need it most can't afford PAT out-of-pocket due to high costs in both finances and time commitments. This is why the Mental Health Parity and Addiction Equity Act (MHPAEA) exists—a piece of legislation enacted in 2008 to ensure that health plans cover mental health treatments fairly, without unnecessary restrictions. In BrainFutures’ latest publication, we cover the basics of PAT and the MHPAEA, potential insurance coverage barriers, and recent federal and state efforts to enforce parity laws. As these treatments become available, it's important that health plans, employers, providers, and patients all understand how the MHPAEA can support full coverage and access for those in need. Case Conferences on New Rapid-Acting Neural-Psychiatric Interventions At Empathy MindED, we are fascinated by the intersection of mental health, obesity, and metabolic changes in the mind and body. In recent years, many patients have started GLP-1 receptor antagonists (GLP-1 RAs) for diabetes and, following FDA approval, for obesity (BMI > 30). There are several things that we are observing with the introduction of these medications: 1) Many patients develop obesity after being introduced to a diet of fast food, soda, and other carbohydrate snacks at an early age. They often consume these sugary snacks during the day while working in stressful environments or living out stressful interpersonal lives. In other words, their obesity was frequently a function of emotional dysregulation combined with increased sugar, fat, and salt in the US food supply. 2) As patients find food less rewarding, they also experience fewer cravings for other sugary snacks, such as cocktails, wine, or beer. 3) Finally, some patients give up highly addictive substances such as nicotine within a week without any intention or effort after starting this class of drugs. In the coming weeks, Empathy MindED will host a series of case conferences on GLP-1 RAs and their use as brain-body medications. This series on GLP-1 RAs will be tailored to psychiatric prescribers and other professionals so that as the science develops, more patients will have access to these treatments. These advancements are crucial in addressing the current limitations in mental healthcare system capacity and expanding access to personalized treatments for individuals. However, integrating LLMs into clinical psychology, particularly in psychotherapy, presents significant challenges due to the need for responsible and evidence-based therapy that requires nuanced expertise. This paper provides a roadmap for the responsible application of clinical LLMs in psychotherapy, covering technical aspects, integration stages, potential applications, and recommendations for development and evaluation. By centering clinical science, fostering interdisciplinary collaboration, and addressing issues like assessment, risk detection, transparency, and bias, LLMs can contribute to a new generation of evidence-based interventions at scale, challenging existing assumptions about psychotherapy. #LLM #Healthcare #BehavioralHealthcare

In the April Perinatal Mental Health & Well-Being issue, Jessica Harrison of the University of California, San Francisco provides an overview of perspectives of interprofessional clinicians on mental health treatment within perinatal health care settings. She finds that there are notable structural constraints to integrating perinatal mental health. Perinatal health clinicians face limited time in the clinical encounter, a lack of mental health competence, and high patient volume. Harrison recommends better mental health care team staffing within perinatal health care settings, the development of educational modules, and increasing the time of clinical encounters. Despite federal legislation improving healthcare access, concerted efforts are still needed to increase evidence-based treatment for maternal perinatal mental health issues, a large study of commercially insured mothers suggested. It found that federal legislation had variable and suboptimal effect on mental health services use by delivering mothers.
