In a report to the Connecticut Aging and Human Services Committees from earlier this year, in fiscal year 2022, from 2004 to 2022 the total number of licensed nursing facility beds declined by 21% (from 29,801 to 23,547). And, during that time, an estimated monthly average of 12,8494 were occupied by Medicaid clients and 6,581 beds were occupied by non-Medicaid clients. https://loom.ly/ufo9kbA This tells me that there is a massive overreliance on Medicaid to provide long term care services.

In this study, the OIG will assess the accuracy of the patient assessment data used to calculate nursing home fall rates. The OIG will use claims data to identify hospitalizations due to falls with major injury among nursing home residents that are Medicare enrollees, including people who are dually enrolled in Medicaid, and then use patient assessments to assess the extent to which nursing homes reported those falls. In April, the Centers for Medicare & Medicaid Services (CMS) released a final rule implementing a minimum staffing standard to address poor-performing nursing homes. This rule requires facilities to provide a minimum of 3.48 hours of direct care per resident per day (HPRD). Though many nursing home residents will likely need care that far exceeds the 3.48 HPRD. If nursing homes wanted to truly care for their residents, they would step up their staffing game. In over half of states, fewer than one-quarter of nursing facilities currently meet all three staffing minimums required in the final federal rule that the Centers for Medicare and Medicaid Services (CMS) released in April, according to our new analysis. The share that does range from 5% or lower in four states (AR, TN, TX and LA) to 50% or higher in five states (AK, ND, ME, HI and OR) and DC. Our new analysis also finds that rural nursing homes are as likely as urban facilities to meet the final rule’s requirements based on current staffing levels, but rural facilities will have longer to comply with the new requirements: https://bit.ly/4auBLRC
