I used the bank holiday weekend in the UK to listen to the International Foundation for Integrated Care (IFIC) Plenary 3 at #ICIC24, co-chaired by Professor Robin Miller and Sarah McLaughlin. The discussion centred around global experiences of general practice in delivering #integratedcare. As Professor Robin Miller challenged in his opening - in general practice we can see both the best and the worst of care integration. Starting with the best, Sarah McLaughlin from Mid & East Antrim Agewell Partnership talked to Billy Johnson about his lived experience of #personcentredcare that was based around what mattered to him and the confidence that gave him in his #generalpractice. It was such a lovely conversation to listen to between two people who clearly understood the value of #relationalcare. Amarjit Maxwell shared the experience of the one of the Primary Health Organisation THINK Hauora in New Zealand as they support the general practice journey to well-being partner for communities, Richard Lewanczuk shared the experience in Alberta, Canada as general practice moves to an extended team that includes people and care-givers as the main providers of care, and Jan De Maeseneer took us on a journey through 50 years of primary care development in Flanders towards Community Health Centres. So many lessons shared along the way. My takeaways: 📈 The pressure on general practice everywhere is increasing and unlikely to decrease anytime soon 🏘 General practice that is moving away from the 'traditional model' of reactive care to preventative and relational care will thrive and help their communities thrive BUT - the 🐘 in the room was 💶! - Investment in new models of multi-disciplinary primary care can accelerate this process hugely, as the evaluation of a model of care shared by Jan shows [https://lnkd.in/eHNmBsux] but this has not yet happened to any significant degree. Expanding Community Involvement in Palliative and End-of-Life Care Conference My key take home messages: 🌟 The importance of "linking up Norfolk with the end in mind." 🌟 Reframing "difficult" conversations as "important" conversations. 🌟 Compassionate Communities - people with a palliative diagnosis or in the end of life phase will spend, at most, 5% of their time with healthcare professionals. What are we doing as a community to support the 95%? 🌟 The importance of access to timely support for loved ones - reassurance and validation. 🌟 Grief is the price we pay for love - holding space to support people is essential. 🌟 Our memories associated with music and sound are important audio biographical cues - helping us to connect and find comfort. And the most powerful message from today's organiser Dr Guy Peryer on how talking is sometimes perceived in healthcare (and society) as the "soft stuff" - "Without the soft stuff, the hard stuff gets harder."
