Those working in aged care and community services can hear from Forgotten Australians and care leavers at this forum about how to provide more appropriate care for those who lived in orphanages, missions, group homes, foster care as children - ageing can mean reinstitutionalisation and some say they would rather die than to lose control and choice about where and how they live…we can do better #forgottenaustralians #careleavers #traumainformedcare #agedcareaustralia

I used the bank holiday weekend in the UK to listen to the International Foundation for Integrated Care (IFIC) Plenary 3 at #ICIC24, co-chaired by Professor Robin Miller and Sarah McLaughlin. The discussion centred around global experiences of general practice in delivering #integratedcare. As Professor Robin Miller challenged in his opening - in general practice we can see both the best and the worst of care integration. Starting with the best, Sarah McLaughlin from Mid & East Antrim Agewell Partnership talked to Billy Johnson about his lived experience of #personcentredcare that was based around what mattered to him and the confidence that gave him in his #generalpractice. It was such a lovely conversation to listen to between two people who clearly understood the value of #relationalcare. Amarjit Maxwell shared the experience of the one of the Primary Health Organisation THINK Hauora in New Zealand as they support the general practice journey to well-being partner for communities, Richard Lewanczuk shared the experience in Alberta, Canada as general practice moves to an extended team that includes people and care-givers as the main providers of care, and Jan De Maeseneer took us on a journey through 50 years of primary care development in Flanders towards Community Health Centres. So many lessons shared along the way. My takeaways: 📈 The pressure on general practice everywhere is increasing and unlikely to decrease anytime soon 🏘 General practice that is moving away from the 'traditional model' of reactive care to preventative and relational care will thrive and help their communities thrive BUT - the 🐘 in the room was 💶! - Investment in new models of multi-disciplinary primary care can accelerate this process hugely, as the evaluation of a model of care shared by Jan shows [https://lnkd.in/eHNmBsux] but this has not yet happened to any significant degree. Let’s create a community of providers who support each other in our work. Here are 3 ways to start builing community: 1️⃣ Share your stories and listen to others. It's about give and take. 🤝 2️⃣ Resources and advice can be gold. Exchange them freely. 💡 3️⃣ Celebrate wins and support through losses. That's real camaraderie. 🎉 Building a supportive community is key to thriving in the medical field. 🏥👩⚕️👨⚕️ #Medicine #Community #Support #physicianburnout #discassessment #professionaldevelopment #medicine #medicalstudents

This is outstanding. What a great partnership between ukactive and Guy’s and St Thomas’ hospital. Feels like the type of thinking needed to support people to be more active. First in their recovery, but also to better set up the routines and communities to continue onwards. Curious to learn more about the outcomes of the pilot – an inspiring blend of healthcare and community engagement. #healthinnovation #partnerships #sport https://lnkd.in/e4TGZTT2

🤝 Local partnerships are the lifeline of comprehensive care support. They create a network of resources that can address the multifaceted needs of individuals requiring care. 🏥 From my experience, when social care providers, local businesses, and community organizations collaborate, the quality and accessibility of care soar. This synergy ensures that care is not just a service but a community mission. 🌱 Take, for instance, a local clinic partnering with a food bank. Together, they can tackle not just medical issues but also nutritional deficiencies that may hinder a patient's recovery. This holistic approach is vital for comprehensive care. 👥 Encouraging comments and thoughts on this can spark a conversation on how we can strengthen our local care networks. What partnerships have you seen or been a part of that have made a difference in your community? #HealthcarePartnerships #CommunityCare #LocalSupport
