A new study by University of Birmingham has shown that a targeted training programme for healthcare professionals can improve part of the care of patients with atrial fibrillation (AF), a common heart rhythm disorder. The STEEER-AF cluster-randomised trial was presented last weekend at the European Society of Cardiology (ESC) Congress 2024 in a hotline session. Led by Professor Dipak Kotecha, the study found that adherence to existing AF treatment guidelines was generally low across Europe. However, a short, structured educational programme for healthcare professionals led to a 51% improvement in guideline adherence for heart rhythm control, although not a significant improvement for stroke prevention. Improved adherence to guidelines means better care and outcomes for patients with AF. This was a prespecified subgroup analysis of the BETTER CARE-HF (Building Electronic Tools to Enhance and Reinforce Cardiovascular Recommendations for Heart Failure) cluster-randomized trial, which compared effectiveness of an alert vs message vs usual care on prescribing of mineralocorticoid antagonists (MRAs). The trial included adults with HFrEF seen in cardiology offices who were eligible for and not prescribed MRAs. Visit volume was defined at the cardiologist-level as number of visits per 6-month study period (high = upper tertile vs non-high = remaining). Study showed that An EHR-embedded alert increased prescribing by >5-fold among patients seen by high-volume cardiologists. Congratulations to our cardiology researchers who have recruited more patients into a multinational heart trial than months they have been open as a recruiting site ❤️ Interventional Cardiologist Dr Aetesam-ur-Rahman and Clinical Trials Assistant Franek Skalbania received special commendation for their success in recruiting patients into the EASY-AS trial. The study, which is being funded by the British Heart Foundation in the UK, is looking to find out whether early heart valve replacement leads to better outcomes after five years in patients with aortic stenosis. Once concluded, the findings could change the way patients with #aortic stenosis are treated. Aortic stenosis is a common and serious heart problem, which causes the heart valve to narrow and not fully open. Symptoms include shortness of breath and chest pain. The Sheffield team are one of 100 cardiology sites across the globe taking part in the trial. I am pleased to announce that our multicenter research letter is now published in Amyloid! We evaluated the correlation between different cardiac tracer uptake and wall thickness on echocardiography. We established that: 1. That there is no correlation between cardiac tracer uptake and wall thickness; 2. Approximately 10% of patients with ATTR-CM have no increased wall thickness, indicating that in patients with unexplained heart failure and “red flags” of ATTR-CM, ATTR-CM should be thought of irrespective of wall thickness; and 3. There is a moderate correlation between LV posterior and IVSd thickness (R=0.69), showing that ATTR-CM can be present in patients with both symmetric as well as asymmetric left ventricular hypertrophy. ATTR-CM can be present in patients with unexplained heart failure and “red flags” irrespective of the presence of (symmetric/asymmetric) left ventricular hypertrophy. We believe that this research letter helps us move forward to ultimately diagnose and subsequently treat patients with ATTR-CM as early as possible.
