Some nurses ask me: "What can I document to cover myself if I know the patient doesn't need more oxytocin, but the provider is pressuring me to turn it up." I review a lot of medical records of babies and mothers that are injured during the birthing process. One very common problem is the mismanagement of oxytocin. I fully understand the pressure put on nurses to increase the oxytocin. I’ve been there before. but… 👏 Nurses👏 if you are doing something that you believe is potentially causing harm, there is nothing you can document to cover your butt. You have a license, and you will potentially have to answer for your actions if they do cause harm. I don’t want you to be in that position. 🔊Please. Please. Please. Do not chart "Pitocin increased per Dr. SoAndSo" when you are also charting recurrent late decels and contractions every 1-2 min. If there is a poor outcome, it will definitely come up in a deposition. If a provider is pressuring you to do something you don’t want to do- learn how to 𝐂𝐔𝐒. These are the 𝐂𝐔𝐒 words: I am 𝐂oncerned. I am 𝐔ncomfortable. I don’t feel like this is 𝐒afe because… If something doesn’t seem right to you, it’s important to raise your concerns, especially when it comes to patient safety. With the 𝐂𝐔𝐒 tool, you can feel empowered to point out a problem or a conflict of information without challenging the other person’s authority or coming off as a personal attack. I’ve found that this tool has helped me navigate a lot of difficult conversations. 🩺 Most providers listen when the nurse says they are concerned and uncomfortable. ✨ Sprinkle in a little safety talk and most are willing to reassess the situation. This includes light snacks, sweets and water. Patients should have an empty stomach during surgery to avoid being sick whilst they’re under anaesthetic.
