One of the best studied care management programs for high-risk patients - the Camden Coalition Care Model - has yielded some fascinating results. The studies have shown that intensive care coordination, while it did increase ambulatory visits both immediately and long-term after a hospital discharge, did not reduce hospital admissions or impact patient outcomes. Further investigation shows that care management that does not attend to social and nonclinical needs is insufficient for complex, high-needs patients. This is important to consider for healthcare systems driven to improve patient outcomes while reducing spend. A recent study featured on HCPLive uncovers a concerning post-pandemic shift in primary care: 1️⃣ Providers are spending 28 more minutes each day on EHR tasks. 2️⃣ Their after-hours workload has increased. 3️⃣ They now spend more time working in the EHR to see the same number of patients. This increase in workload adds a significant risk of burnout. At Abridge, we are committed to providing solutions that help reduce clinician burnout.
