➡️ A major reason maternity units exist and NICUs and other peripheral services are built or by healthcare organizations is so that you have a 'safe place' to deliver and if something goes wrong or unexpected issues arise, there are skilled, trained professionals right there onsite, ready to handle obstetrical and neonatal issues. So why do we find ourselves battling these unfortunate healthcare outcomes related to all this cultural and socioeconomic bias on behalf of the ones who are charged with execution of care??? 😕What does this say about workplace culture-- implicit or not?? 🚫 Moms not wanting to be looked at as 'difficult'. 🚫Lack of education around careplans and appropriate rationales so you can't even advocate for your own care and HAVE to agree to implementation. Or even worse, they tell you plan and don't bother asking for consent (again, so your aren't seen a trouble) 🚫 Being yelled at by the provider??? Is it surprising that social media has become a major source of education for patients and they would rather consume and build relationships there then in the clinic or hospital at the moment that care is recieved?? There are so many sorespots on maternity units and in clinics. We have to fight harder for the trust of our maternal community. They are literally dodging the hospitals out of fear of neglect. There are important lessons to be learned from efforts to address disparities in NY, where Black women are 9x as likely as White women to die in pregnancy/labor. So much attention has been focused on the social determinants of health in recent years — and for good reason — but for methodological reasons, we have likely been underestimating the role that health care can play. As this Times story references, differences in health care quality may explain about half of the Black-White difference in outcomes.

This #BlackMaternalHealthWeek ➡️ Jen, a labor nurse, shares a powerful truth: Black women face a different reality in childbirth. Why? Implicit bias and systemic disparities lead to: *Higher risk: Black women are 4 times more likely to die from pregnancy complications than white women, often due to unconscious bias in healthcare. *Unequal access: Quality prenatal care, pain management, and postpartum support are harder to find in Black communities. We can change this. Jitong Zhao, et al. investigated the effects of social factors including social support and social restriction on postpartum depression (PPD) during the COVID-19 pandemic. They found that social support acted as an important protective factor for PPD during the COVID-19 pandemic and that social restriction increased the risk of PPD. EPDS may be an optimal choice for researchers to define PPD. Obstetricians should underline social factors for both pregnancy women and their families. The 6-week check we know is not fit for purpose, many of the questions i have been asked at my 6-week check have been in relation to contraception and simply if I am feeling ok. Many mothers may not wish to share openly with their GP (who they may not have a relationship with or feel embarrassed to share) how they are feeling. Research shows that one in ten mothers who shared their experience didn’t receive a 6-week check. Of those women who received a 6-week check only one in five were satisfied with the check. Most shockingly nearly half felt that they did not spend enough time talking about their mental health. I know from talking to all the mothers I see every week that their experiences did not differ either if not were worse. Which scares me. Provisions for pregnant women are poor but for postnatal women it is even worse, apart from this 6-week check there is no more support for the mother other than the health visitor visits (which are few) and let’s not forget the health visitors role is to take care of the baby. I personally think mothers should be seen by the midwife for the first 12 weeks of the babies life. As a mother those first few months are hard, this is the time when the mother needs support from a health care professional. Ideally this midwife would be the same midwife from pregnancy to ensure a strong relationship exists and for continuation of care. Postnatal provisions are very important, the mother goes through huge trauma at birth and may be also suffering from pelvic floor dysfunction and Diastasis recti. Many clients I speak to choose to pay for a mummy MOT with a women’s health physio. BUT not everyone can afford this. Within the FIT MAMA app you can access support from women’s health physios in the form of videos and articles, to give information and guidance to those who may need it the most. FIT MAMA app is the solution for better pre and postnatal provisions. This combined with better provisions from the NHS is the perfect solution. I am working hard to work with local trusts in my area to pilot the app with prenatal and postnatal women. A new @HopkinsMedicine study shows U.S.-born Black women are more likely to develop preeclampsia, a serious form of high blood pressure during pregnancy, than their foreign-born counterparts. Perhaps the stress of systemic racism, living in racially segregated neighborhoods and experiences of discrimination are likely contributors to the poor health of Black women and may therefore contribute to disparities in cardiovascular risk factors and preeclampsia.
