In the Designing Equitable Care for Kidneys (DECK) study, we screen for both clinical and social factors. We identify unmet social needs using the PRAPARE tool and organize data into summary domains to identify needs. We’ve formulated these domains into the SHIFT criteria: 1. Safety [interpersonal] 2. Housing stability 3. Insecurity [Food] 4. Financial difficulties [utilities] 5. Transportation Understanding these needs creates an opportunity to create a SHIFT in a person's access to necessary resources. We believe, and prior research evidence supports, that addressing these social determinants of health can significantly impact clinical care outcomes by reducing barriers and improving overall health management and adherence to treatment plans.

Today is World Delirium Awareness Day - an annual event in March to raise awareness about delirium and its impact on patients, families, and healthcare systems. Delirium is described as a serious disturbance in mental abilities that develops rapidly and severely impacts a person's perception, cognition and awareness. At Northern Health, the Delirium and Cognitive Impairment Clinical Improvement Committee is a subcommittee under Standard 5 Comprehensive Care. The purpose of the committee to provide governance over the development or adoption of a system that uses best practice evidence - based improvement strategies in service delivery and minimises harm to patients who have cognitive impairment or are at risk of developing delirium. #wearenorthern #safekindtogether #yearofkindness #deliriumawareness #northernhealth
