Australia-based Neuren Pharmaceuticals Limited is advancing NNZ-2591 for several severely debilitating neurological disorders with no or limited approved treatment options. In December 2023, Neuren reported encouraging outcomes from a Phase 2 trial focused on Phelan-McDermid syndrome. The company anticipates releasing top-line results from a Phase 2 trial investigating NNZ-2591’s efficacy in Angelman syndrome in Q3 2024. You can only feel pain, but can’t measure though… eh, Little too philosophical?? Well it’s a reality for clinical studies in pain. Regardless of the how it started, why or when, a researcher may only ask the patient – “How do you feel or rate your pain” … till date there is no “accepted” measurable outcome!   Is it a challenge? A patient’s response is usually a mix of “what treatment did” and “what patient expected it to do” – and it’s a subjective one😊.   As a Result – Often, patients on placebo or non-efficacious treatment report improvement in pain symptoms!! so called “placebo-responders” – impairing ability of a study to differentiate a drug which works! Over years many remedial approaches have been proposed (incl. crossover design, patient-training), none of which could consistently address all situations.   Cut to chase – Back to my days at Bayer… we thought – “How about we detect such patients early based on pre-treatment responses?” But how? crudely speaking, match patients to usual traits of a “placebo-responder”, using patterns derived from historical studies. While exploratory, we were able to tease out some important and meaningful patterns of “placebo-responders” which enables a probabilistic model-based identification. And its my immense pleasure to share with you that the proposed approach has now been accepted in one of the most prestigious journals on pain. My sincere gratitude to the whole study team and co-authors on this journey for their support and a special Thanks to Brigitte Stemper who not only led the efforts but has always encouraged me to explore new methodologies.    

1. **Study Overview**: A year-long trial investigated the effects of liraglutide, a drug for weight loss and diabetes, on cognitive decline in individuals with mild Alzheimer’s disease. 2. **Results**: Participants receiving liraglutide showed an 18% reduction in cognitive decline compared to those given a placebo. 3. **Expert Opinions**: Neurologist Alberto Espay noted the potential of the drug to become a groundbreaking treatment if results are replicated in larger trials. 4. **Need for Further Research**: Experts emphasize that larger studies are necessary to confirm these preliminary findings, which contribute to the exploration of diabetes and weight loss medications for Alzheimer’s treatment. 5. **Mechanism of Action**: GLP-1 agonists, like liraglutide, may reduce amyloid protein levels in the brain, which are associated with Alzheimer’s. 6.Funding and Future Trials**: The study was partially funded by Novo Nordisk, which sells liraglutide. Ongoing larger trials are investigating other GLP-1 agonists, like semaglutide, in treating Alzheimer’s. 7.Significance**: If proven effective, liraglutide could significantly change the treatment landscape for Alzheimer’s disease due to its safety profile and potential efficacy.

Transcranial Direct Current Stimulation (tDCS) is a fairly common practice technique used to stimulate and regulate cortical excitability - essentially the degree to which our neurons react to a given stimulus. Kong et al., (2024) recently submitted a paper on the efficacy of this tDCS in treating anhedonia in patients suffering from MDD. Moreover, they go so far as to discuss and compare the efficacy of the treatment, localised to specific transcranial regions. A fascinating read.

A double-blind clinical trial of 120 patients showed that topical use of Nigella Sativa has significantly better effects than synthetic drugs and placebo. Symptoms such as numbness, burning, and muscle cramps were significantly reduced. This natural option could be an effective solution without the side effects of conventional drugs. The results are promising for improving the quality of life of diabetic patients. Ophthopedia Update:Metformin in Glaucoma Treatment: Purpose: The objective of this study was to evaluate the potential beneficial impact of metformin on glaucoma risk factors and explore the underlying mechanisms. The goal is to contribute to the development of novel treatment strategies for glaucoma. Methods: We searched for studies that assessed the effects of metformin on glaucoma risk factors and the associated underlying mechanisms. Our search included electronic databases such as PUBMED, EMBASE, and clinicaltrials.gov. Results: Unfortunately, we did not find any clinical trials that specifically investigated the impact of metformin on glaucoma. However, data from experimental studies demonstrated the capability of metformin to modulate various pathways that could contribute to neuroprotection in glaucoma. Conclusion: In order to develop comprehensive treatment strategies that fully utilize the therapeutic potential of metformin in glaucoma management, rigorous clinical studies are imperative. Kudos to the The National Institute on Drug Abuse (NIDA) and FDA on their published correspondence in Springer Nature Group / Nature Medicine on recent progress in the development of novel medical devices to combat the opioid epidemic. Neurotype Inc. was honored to participate in the 2022 FDA workshop discussed in the article.
