🌟 It's Election Day! 🌟 Your voice matters, just like your wellness journey. Taking the time to vote is a powerful way to invest in your community, just as investing in your health is a way to empower yourself. We understand that everyone's path to wellness looks different, and the same goes for participating in civic engagement. Whether you’ve already voted, plan to visit the polls today, or are helping others get there, you’re making a difference. 🙌 As a community, we stand for respect, understanding, and the belief that each step forward counts—whether it's a step toward your health goals or toward shaping the future you believe in. 🗳️ Make your plan to vote, stay hydrated, and remember that progress is made one step (or one ballot) at a time!

Healthy Habits: Combating Isolation, Strengthening Communities Social isolation significantly affects seniors’ health and well-being. That’s why the Healthy Habits program prioritizes social engagement—creating a space where seniors can connect, stay active, and feel supported. Just last month, a participant shared how Healthy Habits helped him rebuild his confidence and reconnect with his community. This program is more than fitness—it’s about transforming lives. With the support of partners like UPMC Health Plan and Highmark, we can expand these opportunities and enhance community health for even more seniors. Interested in advancing social health initiatives? Let’s collaborate to support the well-being of seniors. #SeniorHealth #SocialConnection #CommunityWellness #UPMC #Highmark

Person-centred values = values that help put the interests of the individual at the centre of everything you do, giving them control of all choices and decisions made about their lives. In health and social care, person-centred values include: individuality, choice, independence, dignity, respect, rights, privacy and partnership. The key features of person-centred planning are: 1️⃣ The belief that the individual is best placed to decide on their support needs 2️⃣ The care plan is owned by the individual and is written in the first person 3️⃣ The individual has as much control as possible over their choices 4️⃣ The care plan is ‘needs’ led rather than ‘service’ led Read our recent blog post to find out more about the principles of person-centred care and their importance in delivering quality care: ⬇️ https://lnkd.in/erDcHAyx #personcentredcare #personcentredvalues #socialcare #qualitycare

Did you know? An estimated 34% of children's chronic illnesses and developmental challenges are caused by environmental factors. These impacts are not distributed equally. Environmental hazards are disproportionately concentrated among communities of color due to enduring, pervasive racist policies like redlining. These harms disproportionately affect children in lower-income families as well as Black, Latino/a, Indigenous, and other children of color. Improving our children’s health depends on righting these environmental injustices. Together, we can build a healthier, safer, more equitable future for all children. #cehday
