After reviewing the video and comments, it's clear that while failing to maintain a proper lookout (Video 2:30 minutes remaining) was crucial, it was not the only issue. The safety findings highlight that if assessment standards are not applied consistently, outcomes become unreliable. Several safety issues in the report were not addressed: 1. Obsolete GPS: The outdated GPS was never replaced, indicating ineffective maintenance of critical equipment. This was not mentioned as a safety finding. 2. Bridge Layout: The second ECDIS station, located behind a curtain, hindered lookout and bridge resource management (BRM) by obscuring instrument readouts, violating MSC/Circ.982 from 2000. 3. SOLAS Chapter V Reg 15: There is no mention of how the bridge layout failed to facilitate the pilot and bridge team in making a full appraisal of the situation and accessing essential information. 4. VTS Tracking Failure: The VTS tracking failure was noted (page 37) but not addressed, as a single point failure as a safety issue. These above indicate that BRM was doomed from the start long before the GPS issue failed as no effective ‘ALL round All Available means could be achieved’ and its long standing. To improve safety, AMSA will need to mandate independent check pilots, separate from pilot companies, across all Australian ports. This would address inconsistent application and enhance reliability and continuous improvement, as well as the current IMO rules above. #AMSA #AMPI #IMPA #ATSB

Our Great Barrier Reef is one of the most biodiverse and important ecosystems on the planet. The more than 10,000 ships that transit it annually are a lifeblood for the Australian economy. Highlighting where any of the multiple layers of defences to keep it safe have not been as effective as designed, and pointing to where improvements can be made, is an important part of the ATSB's role. Today we release our final report from an investigation into a serious incident where a bulk carrier came within 200 m of grounding in the Great Barrier Reef after a GPS unit began providing false information to the pilot and crew on board. https://lnkd.in/gyFhTiX2
