Addressing Maternal Overdoses in Montana The opioid crisis is severely impacting pregnant women. Since 1999, the opioid mortality rate among women has surged by over 500%, with opioid use disorder more than quadrupling. Unintentional drug overdose has become a leading cause of death among pregnant and postpartum women. Despite these alarming statistics, only 6% of Montana’s SUD facilities have programs for pregnant and postpartum women. At SIM-MT, we’re stepping up to address this gap with lifesaving training programs. This young lady recently died from cervical cancer.  Tragic incidents like this underscore the urgent need for improved patient-provider communication in healthcare. As healthcare providers, it's our responsibility to prioritize active listening, acknowledge patient concerns, and work collaboratively to address their needs. We must create environments where patients feel empowered to advocate for themselves, seek second opinions, and engage in open dialogue about sensitive topics like abnormally heavy periods, contraception, and reproductive health. Let's commit to fostering trusting relationships with our patients, valuing their input, and ensuring that their voices are heard and respected.Moreover, it's crucial for healthcare providers to take the time to understand their patients' beliefs, desires, and unique circumstances. This woman declined medical treatment in lieu of more holistic options. I'm not sure if she verbalized this to her provider, but I can't help but think she needed more education to understand the severity of her situation. She could've taken the holistic route in addition to modern medicine. By incorporating patient preferences into treatment plans and providing comprehensive education about their options, we can empower individuals to make informed decisions about their health. That antidepressant dispensing rates accelerated so rapidly after the start of the pandemic is not surprising. What is surprising is that this increase occurred even though antidepressant dispensing to male adolescents significantly declined. Fascinating but also concerning findings.

It's disheartening to acknowledge that many substance use disorder treatment programs still present significant barriers for pregnant women, particularly in their third trimester. The additional challenge arises for mothers who, post-birth, face the difficult decision of seeking treatment while potentially being separated from their child. The need for policies supporting family treatment programs is urgent. Pregnant and parenting women deserve access to life-saving treatment without the fear of separation from their child. These programs should provide the essential prenatal and postnatal care crucial for the well-being of both mother and child. Let's work towards a future where no woman is denied the care she needs during such a vulnerable time. The cost of not doing something is far too great. An MSNI report into the incident concluded that... ⚫Ethnicity and health inequalities impacted on the care provided ⚫Cultural bias and stereotyping delayed the care given ⚫An unconscious cultural bias delayed the timing of diagnosis and response to her clinical deterioration ⚫The report added the above was evident in discussions with staff involved in the direct care of the patient. #inclusionanddiversity #healthequity
