About us

ConcertoCare is a tech-enabled, value-based provider of at-home, comprehensive care for seniors and other adults with unmet health and social needs. NOTE: We are aware of individuals and entities fraudulently representing themselves as ConcertoCare recruiters and/or hiring managers. ConcertoCare will never ask for financial information or payment, or for personal information such as bank account number or social security number during the job application or interview process. Any emails from the ConcertoCare recruiting team will come from a @concertocare.com email address. ConcertoCare deploys physician-led interdisciplinary teams— supported by its proprietary population health platform, Patient3D®, and clinical decision support tools — to manage the country's most medically and socially complex and costly patients in ways that keep them in their homes and out of the hospital. The results are better health outcomes, reduced costs, and a more personalized and integrated experience for patients and their families. ConcertoCare works with a wide variety of payers and can partner with a patient’s current primary care physician or serve as the provider of record. Led by a world-class team of home-based care physicians, population health technology experts, former policymakers, and senior health plan executives, ConcertoCare serves seniors across the country. ConcertoCare’s value-based, in-home care model is transforming support for Medicare and Duals members while delivering health plan cost savings. For payers, an investment in ConcertoCare's program pays dividends through reduction of unnecessary ED visits and admissions, prevention of readmissions, and reductions in the need for costlier institutional care. It's a win-win for health plans, their members, and the health system at large.

According to the National Council on Aging, LGBTQ+ older adults have higher rates of chronic conditions, such as cancer and heart disease. At ConcertoCare, we place health equity at the center of our care model and rigorously work to identify and address social determinants of health (SDOH) that may lead to such disparities. https://bit.ly/3oCV1dU

In recognition of #Juneteenth, we are encouraging our community to take time to educate themselves about the history of this day. On June 19, 1865 -- 2 months after Robert E. Lee's surrender at Appomattox, 2.5 years after the Emancipation Proclamation, and nearly nine decades after Independence Day -- the last enslaved African Americans in Texas were finally informed they were free. Now a Federal Holiday, Juneteenth is an annual reminder of our shared history and an opportunity for us to honor African American freedom.
