Case of the Week A patient diagnosed with Double Outlet Right Ventricle (DORV) and multiple Ventricular Septal Defects (VSDs) presented a challenging scenario for surgical intervention. Confronted with the decision to undertake biventricular repair, the surgical team needed a deeper understanding of the patient's heart, surpassing what could be obtained through 2D imaging alone. “Occasionally, slight disparities arise between echocardiograms and CT scans interpretation. Roux-en-Y Biliary stenting in bile leak An interesting case of post-operative bile leak, managed by percutaneous and endoscopic intervention. 54yr gentleman had undergone cholecystitis for GB stones. Postop he developed bile leak. ERCP was tried, but due to complete transection of CHD, it was not possible to cannulate IHBR. So percutaneous access of right hepatic duct was done with micro puncture set. Wire was passed through CHD rent into collection. Using snare, this wire was caught from down with ERCP. Wire was brought outside from mouth. Then over the wire plastic stent was placed. Intravascular Ultrasound (IVUS) IVUS is a diagnostic procedure using sound waves to visualize the inside of blood vessels, particularly coronary arteries. A catheter with a tiny ultrasound wand is inserted through the groin, providing an inside-out view of artery walls. This helps evaluate the need for heart surgery, treat certain heart conditions, and assess stent placement during angioplasty. IVUS reveals cholesterol deposits and helps understand stent restenosis. Risks include general anesthesia complications and potential damage to heart structures. After the procedure, a brief hospital stay may be required, depending on whether it was done during cardiac catheterization or angioplasty. This weeks case of Vascular Vriday/Fascular Friday 🔥 +70 female, 2 years after aortic endoprothesis. Vascular acces right groin closed with Manta-plug. Now presenting with large pulsatile hematoma in the groin . CT scan revealed a false aneurysm in the distal CFA 🔦 Right above the femoral bifurcation, no real adequate sealing zone 🙀 Possibly long-term low-grade infection of the groin. Patient not fit for surgery. What to do? We tried placing a 8 mm W. L. Gore & Associates Viabahn stent in the CFA, but didn't seal, as expected. Placed an Amplatz-plug in the deep femoral artery and extended Viabahn from external iliac artery to superficial femoral artery to seal this deal 🔒 How would you have solved this case? ON SEPTEMBER 16, 1977, DR. ANDREAS GRUENTZIG used a balloon to dilate a severe stenosis in the coronary artery of a conscious patient. Relief of angina was afforded by a procedure using a catheter that had been inserted into the patient’s leg artery and advanced to the heart. At that time, the only procedure available to treat medically refractory angina was open-heart, coronary artery bypass graft surgery, also known as bypass. The Swiss postman who was the first patient to receive coronary artery balloon angioplasty (plasty), had been scheduled for bypass. His cardiac surgeon was standing by in case the prediction “inflating a balloon within the diseased arteries of a human heart would be like throwing a hand grenade into a dumpster” occurred. It did not. That fateful day, the surgeon’s services were not required. The RHEIA trial, presented at ESC Congress 2024, found that transcatheter aortic valve implantation (TAVI) is superior to surgical aortic valve replacement for women with severe symptomatic aortic stenosis. TAVI reduced the combined risk of death, stroke, and rehospitalization by 6.8% compared to surgery. It also led to fewer rehospitalizations and a shorter hospital stay, although it was associated with a higher rate of new pacemaker implantation and mild aortic regurgitation.
