In the lead up to National Advance Care Planning Week, 18 to 24 March, we’re putting the spotlight on the Advance Care Planning Program St Vincent's Hospital Melbourne. This program was established in 2013 and aims to encourage patients to appoint a Medical Treatment Decision Maker and talk about personal values and preferences regarding their health care. St Vincent’s document these in their medical record and in legal forms. Caroline Scott, Manager of the Advance Care Planning program at St Vincent’s Hospital Melbourne says, ‘at St Vincent’s Hospital Melbourne, it’s very important for us to know the values of our patients, so that we can provide the care they want when they are unable to speak for themselves.’ These conversations can be challenging but important, and starting a conversation can assist people in determining what living well means to them and who they would like to make medical treatment decisions for them in the event they are too unwell to do this themselves. Ideally these conversations lead to documenting an Advance Care Directive and nominating a medical treatment decision maker who ensures these preferences are made known. The Advance Care Planning Program at St Vincent’s provides this service to admitted patients. Sometimes it can be hard to know what to say. “For me, doing the research and finding the proof is not enough. Once we have found the research, or once the research has been developed and created and is out there for everyone to see, then we need to understand how we can incorporate that research into evidence based, high quality care.” EMF Board Director Associate Professor Viet Tran spoke to Australian Health Journal about emergency medicine as a specialty, pathways to become an emergency physician, and what research means to him in the context of delivering high quality patient care. “Emergency medicine research forms one of the four key pillars to delivering high quality patient care. These pillars are research, education, quality and safety. With research, what you have is being able to answer how or what high quality care is. The education is about how many people can deliver it, whereas quality is about how systems can deliver what the research has shown. And then safety is about how systems can minimise the harm provided if the right context isn't applied for application of that research.” A/Prof Tran joined the EMF Board in 2020 to further the passion and culture of research that EMF has cultivated within the emergency care community. Interesting letter from the American Hospital Association pushing back on the excellent oped highlighting the challenges of hospital tax-exemption by my colleague and fellow policy researcher Amol S Navathe in the The New York Times. A few observations about the industry letter: -The hospital industry has over used the term "cherry picking." It is a tired term used whenever anyone says something that makes industry look bad. -I don't think that Amol is cherry-picking statistics, the literature on hospital tax-exemption and community benefit is pretty uniform and clear in its direction...and uniformly looks bad for the industry (the debate is the magnitude). -The argument about uncompensated care is much weaker a decade after the ACA has passed which prompted (1) the creation of ACA exchanges and (2) Medicaid expansion -The extreme stress from the COVD-19 pandemic was arguably exacerbated by the administrative staff heavy model of health systems...industry should probably stop using the pandemic argument... (see stories about patients dying alone in the hospital isolated from family) -The industry noted that hospitals provide maternal and prenatal education. With US maternal mortality being (frankly speaking) absolutely terrible is this something that industry really wants to take credit for? -Industry also noted that hospitals provide transportation, housing, and economic opportunities. From a public policy perspective, this is the equivalent of asking SpaceX to design my new reading glasses...maybe not the best allocation of corporate focus and strategic effort. -No mention is made of the hard-working 1,200 tax-paying hospitals that do not benefit from the numerous subsidies that tax-exempt hospitals have. Many tax-paying hospitals are located in rural and poor areas too.
