In safety, the Iceberg Theory illustrates how visible incidents are only a small part of a larger issue. For example, the visible part of the iceberg might be a workplace accident or near miss. Below the surface lies a more extensive set of underlying issues, such as systemic problems, unsafe practices, or a lack of training, which contribute to the visible incidents. The theory emphasizes the importance of addressing these hidden issues to prevent future accidents and improve overall safety.
