🚀 Championing the Future: The Rise of Hospital at Home A recent article in The Wall Street Journal has cast a spotlight on the expansion of hospital at home programs, with some suggesting that there’s resistance from policymakers. Let’s take a deeper dive. The term “policymakers” in the article actually refers to a report from the Medicare Payment Advisory Commission (MedPAC). While MedPAC advises Congress on Medicare payment issues, it does not create policy. In fact, MedPAC’s report highlights the potential advantages of hospital at home and the safety of it for Medicare beneficiaries. It’s not a question of opposition but an opportunity for Congress—the real policymakers—to leverage valuable feedback from MedPAC to strengthen and expand hospital at home care. Moreover, the Bipartisan Policy Center’s recent report supports a five-year extension of the hospital at home model, reinforcing the momentum and signaling strong support for its future. With these insights and endorsements, Congress is poised to make strategic decisions that will drive the success of hospital at home initiatives. The NHS is asking the public to still come forward as usual for care during collective action by GP services which starts today (Thursday 1 August). GP services across England will begin collective action on Thursday 1 August for an indefinite period of time, and while there may be some disruption to services practices will remain open as usual and patients can make requests by phone, on-line or by walking in. During collective action, practices are still required to be open between 08:00 and 18:30 Monday to Friday. It is vital that patients still attend their appointments unless they are told otherwise, and practices should inform you of any changes to services. The nature of the action means that the impacts will vary at different GP practices and from area to area but could include GPs limiting the number of patient appointments per day. NHS teams have worked hard to plan for disruption and to mitigate this where possible to ensure services continued to be provided for patients. Patients should continue to use 111 for urgent medical help when their GP practice is unavailable and if it is a serious or life-threatening emergency call 999.

MBI welcomes the findings of the report The Hidden Waitlist: The Growing Follow-up Backlog, published today by Reform Think Tank, Sebastian Rees, Hashmath Hassan, which estimates that the total number of people currently waiting for follow-up appointments at acute Trusts in England stands at more than 11 million.   “The report highlights the need for greater transparency of the follow-up backlog. Whilst the total size of the list will get headlines, the real question is: how many of the total are overdue? The gaps in information are concerning, but perhaps inevitable given the polarised focus on one set of patients over another. We fully support the report’s main recommendation to report on these patients regularly.   It is only through full transparency of patient demand and visibility of all waiting list activity that hospitals and the NHS can properly inform patient safety, recovery planning and direct precious resource.   With patients increasingly suffering negative outcomes from long waits or missed follow-up appointments, it’s essential that the NHS has the data to prioritise all of its patients, not just RTT. We need to understand the competing demands between these lists and focus on patient care and risk. Transparency and better data are the basis for doing this.   The report raises some key policy issues in how elective care patients are being managed in different ways depending on which list they are on. If the legislature can be extended and enough use cases created to prove the concept so that all Medicare Beneficiaries have access to the "hospital-at-home" model beyond 2027, then not only will the cost of care tremendously decrease at hospitals, but it will provide significant solutions to overcrowded hospitals, increase reach and access to rural areas, decrease hospital readmissions, and improve patient's overall quality of life. Yes, there's still a lot to figure out, primarily the billing concepts, as this is not directly a traditional Medicare Part A Service; perhaps the MLTCs can pick up on this? As someone who had spent much time caring for an ailing family member in their final transition, I have seen tremendous benefits of this concept - primarily making their final time easier, kinder, and more humane - and it gave our family something that can never be taken for granted, seeing more smiles and peace on the family member's face when they were at home - rather than the agony, fear, contempt, and delirium when they were in the hospital.
