📊 The incidence of Placenta Accreta Spectrum (PAS) is increasing globally. With a mortality as high as 7%, (1) PAS is a serious pregnancy complication where the placenta abnormally attaches to the uterine wall, often causing severe bleeding and other dangerous complications. This underscores the importance of improving access to quality maternal healthcare worldwide. They say that an ounce of prevention is worth a pound of cure. In aggregate, that's true, and demonstrates very nicely the overall mission of public health. In some cases though, the value of prevention is so high relative to that of cure that, rather like dividing by zero, the ratio is incalculable. In such situations, even one case of illness represents a failure of the system. Congenital syphilis (CS), the infection of an infant by an improperly or untreated mother is one such case. Cooper and Sanchez, in Seminars in Perinatology, say that EVERY case of congenital syphilis is a failure of the public health system. That's because with adequate prenatal testing, up to three tests recommended for at-risk pregnant women, and adequate treatment--injections of bicillin, no infant should have to face the crippling and even deadly consequences of CS. Children born with CS face crippling physical deformity, developmental disability, palsy, blindness, and even death. While they can be treated and cured of syphilis, its effects are often lifelong. The inadequacy of prenatal care in the United States compared to other developed nations is well documented on many fronts. In the face of a rapidly swelling epidemic of syphilis among women aged 15-44 nationwide, however, it is specifically performing inadequately to prevent CS. Across the nation, cases rose 754% between 2012 and 2021, and continue to rise sharply, with 3,755 cases reported in 2022, or 102.5 per 100,000 live births. Among the most vulnerable populations, such as Alaskan Indian/American Indians, the number approaches a staggering 1 out of every 100 live births. Every one of those cases is somebody's precious child, and each case is ultimately preventable with a greater, awareness, effort, and investment in prenatal care. Easier said than done, I know, but we must start somewhere. Prevention is possible, can be relatively painless, and can work every time. Most of us will know someone who has had a pre-term baby and how immensely challenging it is! I’m so thankful that pioneering research conducted in NZ in the late 1960s led the world in the use of antenatal corticosteroids’ for mothers at risk of giving birth early and 50 years on, a new study has evidenced the success!   Between 1969-1974, New Zealand took the helm in global research to study the effectiveness of giving antenatal corticosteroids to over 1,000 pregnant women at risk of giving birth early. The researchers that discovered this innovative treatment, not only found it helped to reduce the risk of breathing problems in pre-term babies, but also improved survival rates.   But that’s not all!    Fast-track to now and Dr Anthony Walters and his team managed to track down 424 of the original babies whose mothers received the late ‘60s/early ‘70s treatment. That’s pretty amazing in my books! 50 years on, 424 of the babies were found as 50 year-old adults, and participated in a new study designed to identify if there were any long-term effects on their health from the corticosteroids given to their mothers, particularly increased rates of cardiovascular or metabolic diseases.   And even better … the evidence from Anthony and his team’s study indicates that exposure to a single course of antenatal corticosteroids is unlikely to have clinically important effects on later cardiometabolic health up to 50 years of age.    Plus the study’s findings suggest the use of antenatal corticosterioids for threatened or indicated pre-term birth provides net clinical benefit in both the short-and-long-term for babies born early.    So all-in-all, at Auckland Medical Research Foundation, we were so delighted to provide Anthony with a Ruth Spencer Medical Research Fellowship so he could undertake the follow-up study and provide continued confidence in the substantial neonatal benefits of using these medications. This is truly world-leading research and you can find Anthony’s published study here https://lnkd.in/gCDprVRZ   Thank you to Perpetual Guardian and the Trustees of the Ruth Spencer Estate for making this work possible!   #MedicalResearch #NeonatalHealth #AntenatalCare #Corticosteroids #PretermBabies #HealthcareInnovation #LongTermHealth #CardiovascularHealth #MetabolicHealth #ResearchFunding

When COVID-19 struck Canada in 2020, crucial knowledge gaps threatened pregnant women's safety. Once it became clear that COVID-19 was primarily a respiratory disease, obstetric alarm bells began ringing because pneumonia – also a respiratory disease – is the third leading cause of death in pregnant women. Dr. Deborah Money, Professor and Head of the Dept. of Obstetrics and Gynecology, UBC, a world-renowned clinician scientist and founder of the from the Women’s Health Research Institute, saw the urgent need for data-driven solutions. With seed funding from iA Financial Group (Industrial Alliance) and exemplary donor support, Dr. Money mobilized a team to rapidly establish a provincial surveillance program to fully understand the impact of COVID-19 in pregnancy. This BC surveillance program quickly evolved into the CANCOVID-Preg Surveillance Project, supported by the Canadian Institutes of Health Research. The Lancet Series on small, vulnerable newborns has highlighted the urgent need for standardisation of data, technology, and quality prenatal information to address the high rate of newborns born too small or too soon. Globally, one in every four babies is either ‘born too small’ or ‘born too soon’, leading to 1.9 million stillbirths and 1.4 million newborn deaths annually. The Series presents a new conceptual framework that brings preterm birth, small for gestational age (SGA), and low birth weight (LBW) together under the term “small vulnerable newborns” (SVN).
