Let's talk about supporting family caregivers when the person they care for is in the hospital!
Why?
1. Family caregivers know the patient best and can provide information about the patient and assist with care.
2. Family Caregivers Provide Medical/ Nursing Care after Discharge
3. Gaps in Family Caregiver Preparation Affect Multiple Stakeholders
--At home, lack of family caregiver preparation can lead to complications and hospital readmission
--when a complication becomes a medical emergency. It can even trigger a cascade to dependency and nursing home placement.
--For hospitals, lack of family caregiver preparation can lead to discharge delays, costly post-discharge emergency department visits, and readmissions— affecting hospital processes, workflows, and financial health.
Integrating family caregiver support into hospital care typically requires a paradigm shift for front-line staff from simply documenting emergency contact information and providing discharge paperwork to consistently identifying, preparing, and instructing the person who will provide care at home after discharge.
In the US, the Caregiver Advise, Record, Enable (CARE) Act is now law in more than 40 states and territories. It mandates a partnership with family caregivers.
Hospital staff need to ask the patient if they have a caregiver, record the caregiver's name on the chart, advise the caregiver of transitions and discharges, and ensure the caregiver is prepared to care on discharge.

Tomorrow is the UN International Day of Care and Support #IDCS2024.
At the International Foundation for Integrated Care (IFIC), we have always acknowledged that central role that carers, specifically informal and family carers, play in care integration. Without carers, for many people #coordination of care simply does not happen.
During our International Conference on Integrated Care 2023 #ICIC23 in Belfast this year, our Director of Communications Fiona Lyne chaired a panel discussion with Pat Grogan from Family Carers Ireland, Richard Meade from Carers Scotland, Sharon Anderson who is a Canadian researcher leading a program on care-giver centred care, and Guilia Lanfredi from Eurocarers.
The discussion highlighted the carers experience of poor, indeed absent, involvement in care planning by the formal care sector specifically healthcare and the impact this has on carers and people they are caring for world-wide.
On the International Day of Care and Support, the International Foundation for Integrated Care (IFIC) would like to spotlight the essential role of the informal, and usually unpaid, carer in #coordination and #continuity of care. They are the invisible thread that holds care together. As Sharon Anderson pointed out in Belfast, family carers are expected to be on call 24/7. I am here today with Pentland Medical at the Difficult Airway Society annual conference being held in the Guild Hall, London. We are sharing our draft report on the findings relating to surgical airway device securement. We are advocating for professionals to use a licenced product rather than the unlicenced tapes and ties. The report is a culmination of a years work through surveys, focus groups and freedom of information requests from NHS trusts. Our short life working group met yesterday and the responses from colleagues will be included in the report and the final document will be available to download - watch this space.

This remind me why being present for your clients truly matters.
Yesterday, one of my clients had an accident at her gym and urgently needed medical advice and assistance.
It just so happened that I was boarding a domestic flight when she reached out.
Despite being on the move, I quickly connected with our medical partner to recommend a trusted doctor and prioritize her appointment.
While I was still mid-flight, she managed to see the doctor and receive the care she needed.
Once I landed, the next steps were simple: guide her through submitting and processing the claim to ensure everything was taken care of seamlessly.
Moments like this highlight the importance of being reliable, accessible, and proactive—not just in resolving problems but in supporting the people who trust us with their well-being.
A small effort on our part can make a huge difference in someone’s day.
To me, that’s what this profession is about: being there for our clients when they need it most.

I'm leaving on a jet plane .......................... don't know when I'll be back again.
Leaving the NHS for 10 days to work in Singapore. Great to be with colleagues from Singapore University of Social Sciences (SUSS) launching the Intercontinental Alliance for Integrated Care a new global learning network looking at Ageing and Integration. I'll also be attending the second Asia Pacific Social Prescribing Conferance with colleagues from SingHealth .
I've been reflecting on the current state of the NHS and colleagues who may be leaving. Many GPs of my age are leaving the NHS and like the song they are torn to be leaving the NHS but life outside is more attractive. GP Retention is an urgent issue as highlighted by the Royal College of General Practitioners with the current workforce plan for General Practice looking bleak over coming years.
This editorial in the BMJ refers to a paper linking retention of acute staff doctors and nurses to mortality but also recognises the importance of continuity in General Practice and retaining the workforce. https://lnkd.in/e2-hJfti
We need some urgent action to recruit new GPs and to retain others. Improved working conditions flexiblity in hours and specific programmes are needed urgently as part of the solutions to fix the NHS. In order to deliver the 3 government shifts we need GPs to stay and not to shift !
So I shall be back again returning to the NHS in 10 days time but how can we keep more GPs in the NHS. What are your solutions?
