A huge congratulations to NELFT NHS Foundation Trust for being shortlisted in the Health Tech News Awards 2024 in the category of 'Major Project Go Live'. NELFT decided to take a long-term, integrated approach to virtual wards, uniting a diverse range of stakeholders from acute and primary care providers to community health services and local authorities. By fostering collaboration, NELFT addressed capacity challenges but also lay the groundwork for a more resilient, patient-focused model of care for the future. The trust worked with their supplier to innovate with their existing technology. Real-time visibility dashboards and the integration with the Access Rio EPR demonstrate how the trust has adapted the model to their specific needs. The project has prioritised the seamless transfer of information between systems, for example between the patient flow solution and the EPR. It enables all the necessary information for discharge to be collated in 15 minutes, improving patient flow and reducing bed blocking. The trust expects to increase the number of patients served on a virtual ward by 300 percent, having gone from five to 15 beds within the first month. On Global What Matters To You day, Advancing Quality Alliance (Aqua) are considering the importance of #WMTY24 and #personalised care how it can help improve patient outcomes. “What Matters to You (#WMTY)?” is an international person-centered care movement inspired by a 2012 New England Journal of Medicine article, #SharedDecisionMaking: The Pinnacle of #PatientCenteredCare, written by Michael Barry & Susan Edgman-Levitan. We know, less than 25% of #NHS organisations that we surveyed in the North West have personalised care, or the ethos of WMTY highlighted in their quality strategies, & only 10% had it explicitly identified as one of their #values. We also know that there is a direct correlation between Question 24 in the Inpatient Survey Test & an organisation’s Care Quality Commission rating. The more often patients respond that they have been involved in their care the higher their CQC rating. For those organisations that have a commitment in their strategy, is it translating into action & for those that don't - why not? This makes me reflect on how we should look at things differently to help tackle challenges that we face. I recently attended an interesting NICE - National Institute for Health and Care Excellence workshop which posed the question – ‘What is the one thing you would do differently?’ My response was to mandate the inclusion of the #patientvoice – essentially ‘What Matters To You?’. I think it would have the potential to: • Reduce #clinicalvariation (look at the work that NHS Scotland are leading around #realisticmedicine).  At present, we know perfect care does not lead to perfect outcomes because patient values are not included ( #WMTY). There is widespread evidence to suggest that including #valuesbasedpractice could reduce costs & improve care - think outcomes & experiences not outputs! • Promoting a shared responsibility for staying healthy & preventing ill health has always had a potential gain for some populations. The ‘new’ focus on #personalisedprevention, building on technological advances over the past decade to more easily align data and condition management with AI is an obvious opportunity. To be really successful it will also have to address the wider determinants of health in terms ‘what matters to you?’ which are far wider reaching than just healthcare. •Reduce the cost in claims and complaints – we know it costs the NHS approx. £50m per year in ill informed consent claims & 1 in 7 people have decision regret. If we asked patients #WMTY this would significantly reduce these costs. •Through wider use of #socialprescribing & #healthcoaching to improve flow & access to different services to support people around #WMTY I know there will be people who will say this is too difficult, or will take too long, but with no more staff & no more money, the only part of the equation we can change is what is important to the patient #WMTY24 – surely its worth a go? We would love to hear your thoughts & ideas.

Since 2005, 104 rural hospitals (~2% of all U.S. hospitals) have closed with an additional 600 under the risk of closure. Rural hospitals, particularly independent ones, fear poor financials and a lack of economies of scale will force them to be acquired by a dominant health system that would end services or even shutter, leaving the local community in the lurch. If necessity is the mother of invention, then it makes sense that we'd start to see more or new creative partnerships, JVs, affiliation agreements, etc amongst rural hospitals and providers. I will be fascinated to follow this announced merger b/w 3 rural MI hospitals. If successful, it could serve as a model for many other independent rural hospitals to follow (*if they have additional indepenent rural hospitals in close proximity).

The Ontario Health West (OH West) Central Intake Project aims to provide equity for CT and MRI accessibility across the OH West region. The project is a significant investment for the healthcare system and is expected to enhance patient access to CT/MRI services, simplify the process for routine booking, optimize system capacity and enable future modernizations and innovative changes. Throughout the month of March, SBGHC participated within a key role of the Proof-of-Concept team, which was designed to test patients’ willingness to travel further distances for shorter wait times for medical imaging. Partnering sites that took part in the proof of concept included: London Health Sciences Centre, St. Joseph’s Healthcare London, St. Thomas Elgin General Hospital and South Bruce Grey Health Centre. 🚀 We're thrilled to announce the release of the findings from the MSK Hubs pilot scheme! 🚀 Over the past two years, leisure centres across the country have delivered an incredible 11,785 sessions as part of the MSK Hubs initiative, designed to help people better manage their musculoskeletal (MSK) conditions through physical activity and self-management. 📊 Key results from the pilot: 44% of participants reported noticeable improvements in managing their pain after 12 weeks of sessions. This highlights the vital role community-based activity can play in improving the quality of life for people living with MSK conditions. Our CEO, Ben Wilkins, shared his insights on the impact of this programme: "The MSK Hubs programme highlighted the leisure sector's capacity to deliver consistent and scalable self-management services for individuals living with musculoskeletal (MSK) conditions and wider multimorbidity. This programme, designed through extensive co-creation, has proven its effectiveness in delivering measurable health improvement across communities. Overall, MSK Hubs has highlighted this model of delivery in local communities provides both a return on investment (ROI) for venues, and a social return on investment (SROI) for the nation. We are incredibly excited to be working with the MSK Hubs consortium and wider collaborators to take the learnings to the next steps of roll-out." As we move forward, we call on the Government to invest in scaling this MSK Hubs model to reach more people and help ease the burden on NHS waiting lists. Exploring alternative models for MSK care delivery has opened up new possibilities, including the potential to elevate our profession and support a higher fee schedule. My recent experience with the "care at home" model has shown it to be a promising alternative to traditional brick-and-mortar setups. However, significant constraints still need to be addressed. When care delivery relies on contractors, maintaining consistent clinical quality becomes challenging. The variability in care, poor communication methods, and limited clinician investment leave much room for improvement. While care-at-home models in the U.S. are still in their beta stage, emerging companies have numerous opportunities to refine the process, focusing on care delivery methods, clinician engagement, and a value-based approach to clinical quality.
