ACH commends Congressional leaders for coming to a bipartisan agreement and finalizing funding for the Community Health Center Trust Fund through the end of 2024 as part of the larger funding package released tonight. This funding level constitutes a 10% increase in the rate of funding for the remainder of the year and is a sorely needed step in the right direction. We are exceedingly grateful to the champions for community health centers, as well as Congressional and Committee leaders, for prioritizing funding for the nation’s largest primary care network. While the need across the country – to meet increasing patient demand, to account for historic budgetary and workforce challenges, and to continue to drive excellence in primary health care – far exceeds this number, we recognize and deeply appreciate this investment. While this is very good news, there is more that needs to be done. We need a longer-term funding appropriation so FQHCs carry forward their missions beyond one year (or 9-months). FQHCs are a driving force in providing access to primary care services, achieving high quality clinical outcomes, and implement innovations that reduce costs to treat. FQHCs need guaranteed long-term funding to support carrying out these initiatives, implementing strategic plans, and developing solutions to workforce needs.

Can you help Medway NHS Foundation Trust and Medway Community Healthcare improve their #research services? We looked at the ethnicities of those who took part in research in Medway and compared this to the ethnicities of Medway residents. We found most of the people who come forward to take part in our research are of white ethnicities and for all other ethnicities the numbers are low. We know that some ethnic groups are more likely to get certain health conditions. If those more at risk of conditions are not taking part in research, how do we know the research results will be right for the whole community? Did you know that Michigan is short about 400 primary care doctors? A solution is underway to address this issue and improve access to medical care, especially for the underserved. A residency program, supported by $960,000 in federal tax dollars and an additional $1 million from Oakland County, is training doctors in a unique way. Instead of traditional hospital-based residencies, these doctors-to-be are rotating through Honor Community Health's network of clinics, school health centers, and even a mobile van serving patients who are homeless. 🚑 This innovative approach not only helps train new doctors but also fosters connections with patients in underserved communities, offering them the care they need. MAFP president Dr. Beena Nagappala said this is a dramatically different approach to training doctors, as residencies often take place in hospitals, where young doctors typically see patients for only a short time. Wider Determinants of Health with MHS Homes Workshop ⭐️ This morning we held a workshop in collaboration with MHS Homes which bought together some of our key partners in Medway that work within the wider determinants of health, particularly around education and housing. The aim of this workshop was to start a conversation on how health can move into the Wider Determinants of Health and build a place based approach to narrow the gap of Health Inequalities in Medway. We had presentations from: - Ashley Hook on MHS Homes and Housing - Simon Cook on opportunities with Education - Nikki Teesdale on the Population Health Management Programme and social regeneration - Natalie Goldring on the Public Health data for Medway -Samantha Whybrow on how the Wider Determinants of Health affect Childhood Asthma - Dr Warren Hickson on the Living Labs programme We then had group working sessions to explore the possibilities of working in collaboration with partners within the Wider Determinants of Health, identifying some quick wins and started to identify how we take a place based approach and work in collaboration. The discussion was extremely rich and the energy was palpable, it was so refreshing to have conversations and hear from people who are so passionate about the people of Medway. This is the very start of the collaboration and I am truly excited about the opportunities arising from this workshop. The full value of primary care can be realized only through a model that promotes longitudinal relationships rather than episodic transactions. She proposes a framework for creating primary care value based on the connections among the four foundations of primary care: 1. The People who are the focus of the care (Patients and Families), 2. Primary care teams, 3. Communities, and 4. Health care systems. Does this sound like primary care you would like? *Effective relationships are grounded in knowing the patient as a whole person. *Communication between interprofessional teams, their patients, and their family supports, and providing connections to relevant community partners. *Clinical schedules that are adjusted to allow for more real time communication and care activities, and ongoing communication that allow changes and issues to be rapidly communicated, assessed, and acted upon. *Community connections.
