Having recently been asked to provide information on standardised training for drivers of these specialist vehicles and any specific guidance available, this recent industry safety alert was used to create a presentation slide to aid investigators in mixer rollover incidents. When investigating mixer rollovers, the immediate cause has always been looked at, while the underlying and root causes remain ignored.  According to the Health and Safety Executive, root causes can generally be attributed to failures by management or organisations, e.g. failure to identify training needs and assess competence, low priority to risk assessments, etc. The mixer manufacturers provide safety-critical information for operating and driving the mixer, which often goes unheeded in favour of erroneous information that can contribute to rollovers, drum RPM being an example.  Conducting a thorough investigation can help prevent similar incidents, especially if a severe injury or fatality is involved, utilising legal requirements, codes of practice, industry guidance and available British standards. The investigation is also looking to understand the industry's training requirement for drivers across the ready-mixed concrete sector, and why any available guidance and safety-critical information is not used, regardless of the high number of mixer rollovers. It doesn't take an expert to work out what we must do to improve the industry's safety, but it could take a legal test case. Transport is identified as one of the Mineral Products Association 'Fatal 6', yet no lessons are learned from previous rollover incidents and subsequent investigations.  Which other sector would continue to allow approximately 25 high-potential incidents per year without taking action? Further questions are why the manufacturer's instructions are ignored even though they provide safety-critical information, why industry rollover guidance is ignored, and why incorrect and dangerous information continues to be accepted and promoted across the sector for commercial gain. Add in the road safety impact, and this becomes a staggering omission of legal and moral obligations. Investigation reports only look at the immediate cause, as placing blame seems easier than preventing future incidents.  "To get rid of weeds, you must dig up the root.
