The Novartis Foundation is a non-profit organization based in Switzerland. For over 40 years, we have helped improve the health of underserved populations, initially supporting disease elimination strategies in areas such as leprosy and malaria. Now, we help cities tackle the burning health issues of our time, cardiovascular disease and health inequity. Cardiovascular disease takes almost 18 million lives each year, and its emergence as a key risk factor for COVID-19 complications brings added urgency to addressing it at the population level. We take a population health approach, which means widening the lens from a narrow focus on healthcare delivery to a panoramic vision of improving health in the population at large, ensuring access to healthy lives for all. Our population health approach brings together disconnected data to create insights that help authorities understand the root causes of unequal health outcomes and find the best ways and best partners to remediate those.     With rapid urbanization and an ever-increasing pressure on health systems, it is cities we have to turn to in our quest toward greater population health.   Acknowledging the need for novel solutions to a persistent problem, the Novartis Foundation and its partners have set out to identify the mechanisms behind unequal health outcomes.     New publication! 📰 🏥 Primary care management improvement 🗺 Patient pathway optimization 👨⚕️👩⚕️ Workforce up-skilling The Novartis Foundation identified these three areas as key elements for generating sustainable impact in cardiovascular population health based on a survey of patients, health care professionals and health system representatives in São Paulo, Brazil. As a consequence, a package of solutions was designed, promptly taken up and deployed by the municipal health secretariat, yielding exceptional results. This package of solutions is now already being implemented in other Brazilian cities, and available for all cities in the region interested to implement the successful 𝐂𝐀𝐑𝐃𝐈𝐎4𝐂𝐢𝐭𝐢𝐞𝐬 approach to improve the heart health of their populations.     “We are proud of the pioneering collaboration we were able to build with the Ministry of Health of Senegal and its NCD Division to tackle hypertension and cardiovascular disease in Senegal.’ Last week, Sarah Des Rosiers, our Director for Health Equity, participated in the handover ceremony of the 𝐂𝐀𝐑𝐃𝐈𝐎4𝐃𝐚𝐤𝐚𝐫 initiative, a rich session that acknowledged the major achievements realized over the last 6 years by the Ministry of Health of Senegal and the Novartis Foundation’s implementing partner, IntraHealth International, to fight the growing burden of hypertension and CVDs. 𝐂𝐀𝐑𝐃𝐈𝐎4𝐃𝐚𝐤𝐚𝐫 was initiated in 2018 with the goal to address the growing hypertension burden in the population of Dakar. Today the prevalence of hypertension is at 30%, with almost 1 in 3 adults affected. The initiative’s aim was to improve its early detection in the communities, strengthen the quality of care from diagnosis to care and treatment, and strengthen the collection of data and its integration into the health information system for better decision-making. Under the leadership of Dr. Malick Anne, Chief of the NCD Division and with the presence of Cheikh Touré, the West African Regional Director of IntraHealth and a representative of the health Dpt of the Mairie de Dakar, participants reviewed the major resulted and tools co-created by this initiative. We would like to thank the Ministère de la Santé et de l'Action Sociale (MSAS) and its NCD Division for the tireless work, IntraHealth for their continual engagement and technical expertise, and to all of the health care and tech players who have made 𝐂𝐀𝐑𝐃𝐈𝐎4𝐃𝐚𝐤𝐚𝐫 a reality. With all of the interventions fully embedded in the activities of the MSAS, we are confident that the actions will continue for a durable success. The Novartis Foundation was one of the first funders in the NCDs space in Senegal and we are pleased to see that new players are making hypertension and CVDs a priority – such as JICA, AMREF and PATH.     Brazil, here comes your heart health boost! ❤️ Too many lives lost too early: for the past decades, cardiovascular disease has been weighing heavily on South America’s biggest country. What used to be a burden for individuals had turned into a massive problem for an entire nation. That needed changing. Since 2017, under the leadership of the health authorities of city of São Paulo, the Novartis Foundation has successfully collaborated with a coalition of local partners to uncover and better manage risk factors for heart health. While implementing our CARDIO4Cities population health approach, nine validated solutions across the public health system’s cascade of care were deployed to lower the cardiovascular risk burden for city inhabitants. Seven years down the road and after touching millions of lives, we are excited to enter a new era of heart health for Brazil. We are proud to announce that yesterday, as part of a small ceremony, the BP - A Beneficência Portuguesa de São Paulo, one of Brazil’s leading excellence clusters in hospital care, population health and cardiology, was officially onboarded as collaboration partner and future execution lead for the initiative in São Paulo. This new partnership is a hallmark of our commitment to generating sustainable local impact and long-term health system strengthening. At the same time, and after already having replicated the program in two other Brazilian municipalities, this milestone is a springboard for the further national scaling of CARDIO, as the Beneficência Portuguesa de São Paulo brings powerful catalytic capabilities needed to move the needle on heart health.     I could not have had a better start to the 2024 World Health Assembly in Geneva than attending the #WorldHeartSummit by the World Heart Federation! I had the tremendous pleasure to form a panel together with the brilliant minds of Jagat Narula MD PhD (President elect, World Heart Federation) and Pablo Perel (Lead Scientist, World Heart Federation), where we spoke about the importance of deploying population health interventions to tackle the ever-growing burden of cardiovascular disease. We agreed that 𝐂𝐀𝐑𝐃𝐈𝐎4𝐂𝐢𝐭𝐢𝐞𝐬, the Novartis Foundation’s population health approach, is a powerful and effective way to alleviate the burden of heart disease through real-time data evidence and precise risk factor management.
