As we continue to mark #WorldPatientSafetyDay today, we call on all patients, families and communities to be fully engaged in health diagnostic processes. Reaching a diagnosis involves many iterative steps, including the patient’s initial presentation; history taking and examination; diagnostic testing, discussion, and communication of results; collaboration and coordination; final diagnosis and treatment plan; and follow-up and re-evaluation. Errors can occur at any stage. Given that breaks in communication between providers and patients are some of the most prominent system vulnerabilities resulting in diagnostic errors, people with lived experience can play a crucial role by actively participating in their healthcare, helping to reduce errors and improve the accuracy of diagnoses. Have a look at the tips on the banner and remember that accurate, timely, efficient and equitable diagnosis requires collaboration from all health stakeholders! Together, we can get it right and make it safe! #WPSD24 #PatientSafety #DiagnosticSafety

With the 2024 #WorldPatientSafetyDay theme being “Improving diagnosis for patient safety," the World Health Organization estimates that almost all adults will experience at least one diagnostic error in their lifetime. Errors can happen at any stage of the diagnostic process in any healthcare settings. Diagnostic errors can be broadly divided into three categories: 1. Delayed diagnosis – When a condition isn't identified early enough, potentially due to outdated methods, improper tests, or failure to act on test results. 2. Incorrect diagnosis – When the wrong diagnosis is made leading to incorrect treatment and missed opportunities for proper care. 3. Missed diagnosis – When a health issue goes unrecognized, causing harm and worsening conditions due to lack of treatment. Reaching an accurate and timely diagnosis is a team effort! It requires collaboration among patients, families, caregivers, health workers, healthcare leaders and policy-makers. All stakeholders must be engaged in shaping the diagnostic process and empowered to voice any concerns. We invite you to learn more about the campaign and join the global health community in marking #WPSD24! Together, we can get it right and make it safe! #WPSD24 #PatientSafety #DiagnosticSafety

International Alliance of Patients' Organizations (IAPO) reposted this
Every year on September 17, the World Health Organization marks #WorldPatientSafetyDay, emphasising the essential theme for 2024: “Improving diagnosis for patient safety: Get it right, make it safe!” In this endeavour, patients are crucial partners. They bring insights from their experiences navigating the healthcare system and have a key role in identifying issues and solutions. Effective diagnosis and follow-up care rely on teamwork, continuous dialogue, collaboration, and mutual respect. For more insights on #patientsafety, read our reflections: https://lnkd.in/dkyUkv-G

In marking world Patient Safety Day 2024, we are once again happy to announce our joint initiative with GS1 Healthcare, The European Association of Hospital Pharmacists (EAHP)), International Hospital Federation and the International Society for Quality in Health Care (ISQua) to promote patient safety globally. Patient safety is about ensuring accurate diagnosis and the right care at the right time, preventing harm, and always prioritising patients. Through collaboration between all five partners in this joint initiative, we aim to raise awareness, share best practices, and advocate for policies that enhance patient safety. Join us in this collaborative effort. Share your thoughts, experiences, and ideas on how we can improve diagnosis for patient safety. Use the hashtags #WPSD24 to be part of the conversation. Together, we can make healthcare safer. #WorldPatientSafetyDay #WPSD24 #PatientSafety

Happy #WorldPatientSafetyDay! World Patient Safety Day brings together all stakeholders in the healthcare system committed to improving patient safety. Through the slogan “Get it right, make it safe,” #WPSD24's theme is "improving diagnosis for patient safety". IAPO celebrates this year's theme and emphasises that patient engagement is the number one measure we can take to enhance diagnostic safety. A safety culture that supports accurate, timely, efficient, and patient-centred diagnoses can only be achieved through multidisciplinary collaboration. In this team effort, patients provide critical insights from their lived experiences, enabling healthcare providers to tailor care to individual needs and preferences, reducing errors and improving overall patient safety outcomes. We invite our community to learn more about the theme (see links in the comments), mobilise your network by sharing information on diagnostic safety, and join the global health community in observing World Patient Safety Day 2024. Together, we can get it right and make it safe! #WPSD2024 #PatientSafety #PatientEngagement

As African leaders gather at the 74th session of the WHO Regional Committee for Africa #RC74AFRO, we as the African Medicines Agency Treaty Alliance (AMATA) proudly participated in today's discussions, delivering a crucial statement under Agenda Item 10: Framework for Strengthening Local Production of Medicines, Vaccines, and Other Health Technologies in the WHO African Region 2025–2035. We urged the African Union to expedite the operationalization of the African Medicines Agency #AMA and to swiftly appoint its Director General. This step is vital to ensuring rapid and equitable access to quality, safe, and effective medicines and health technologies across the continent. We also called on all AU Member States that have not yet ratified the #AMA to do so with urgency as a robust continental regulatory system is essential to improving public health and enhancing the quality of life for all Africans.
