About us

PACE is a Medicare program for older adults and people over the age 55 living with chronic illness. This program provides community-based care and services to people who otherwise need nursing home level of care. PACE was created as a way to provide you, your family, caregivers, and professional health care providers flexibility to meet your health needs and to help you continue living in the community. PACE provides all the care and services covered by MediCare and MediCal, as authorized by an interdisciplinary team, as well as additional medically-necessary care and services not covered by MediCare and MediCal. PACE provides coverage for prescription drugs, doctor care, transportation, home care, check ups, hospital visits, and even nursing home stays whenever necessary. With PACE, your ability to pay will never keep you from getting the care you need.

    A few highlights from our 2nd Annual Talent Show at Gary and Mary West PACE ! This talent show brought our senior community together in an inspiring display of showmanship and creativity. Even our therapy team got into the "act" and kicked off the show with their "Safety Dance"- steps infused with humor and love. We are grateful to everyone who contributed to making this event a success.     The Missing Man Table. Each Item on the Missing Man table represents the emotions and feelings reserved for those who did not come home. -The table is round, to show our everlasting concern for our missing men. -The cloth is white, symbolizing the purity of their motives when answering the call to serve. -The single red rose; displayed in a vase, reminds us of the lives of these Americans and their loved ones and friends who keep the faith while seeking answers. -The red ribbon symbolizes our continued determination to account for our missing. -A slice of lemon reminds us of their bitter fate; captured and missing in a foreign land. -A pinch of salt symbolizes the tears of our missing and their families who long for answers after decades of uncertainty. -The lighted candle reflects our hope for their return, dead or alive. -The Bible represents the strength gained through faith to sustain us and those lost from our country, founded as one nation under God. -The glass is inverted, symbolizing their inability to share a toast. The chair is empty, the seat that remains unclaimed at the table. In memory of many, in honor of all, thank you. Your service and bravery will never be forgotten. #MemorialDay #missingmantable

    May is Mental Health Awareness Month. As our population continues to age, it's crucial to address the complex healthcare needs of our seniors, especially regarding mental health. Did you know that 80% of older adults have at least one chronic disease, and a staggering 68% have at least two? Additionally, a significant portion of older adults aren't receiving the behavioral health care they require. PACE is a revolutionary model of care designed to provide seniors with comprehensive healthcare services, allowing them to thrive independently within their communities. This includes essential mental health services that are often overlooked or inaccessible to many seniors.
