We work to solve the fragmentation of healthcare. We are a team of highly experienced clinicians and technologists that have come together on a mission: SIMPLER and BETTER health care, through one digital platform that connects everyone to enable patient success. Why? Health care is already highly fragmented. New technologies that are being added fragment the health experience even further. This impacts the most patients with complex or chronic health issues, as gaps and overlaps are the main drivers of care dissatisfaction, poor health outcomes and skyrocketing health costs. How? Careteam's digital collaboration platform ensures that patients, families and health professionals manage health issues proactively and are on the same page. This improves the experience and outcomes of care while optimizing costs and use of resources. Careteam is a powerfully simple, cloud based and AI-enabled digital platform that enables care planning, communication and task management, and offers a set of tools that integrate the other technologies that people already use. Careteam enables clinicians to create care plans for patients with complex and chronic health issues that include instructions and recommended resources. The patient or their main caregiver can then share the care plans with who they see as their circle of care and Careteam enables the activation of the care plan into a to do action plan. Health organisations can monitor whether the plan is being actioned and proactively intervene to prevent deteriorations. Built by clinicians and patients and sold to health organisations focused on scaling innovation and accelerating their move to patient-centered care in the community, Careteam the only system that flexibly adapts to all situations and offers patients full control of who on their support network gets involved.     Pierre de Coubertin's quote, "The Olympics are a celebration of the human potential and the triumph of the human spirit," symbolizes more than just athletic, mental and physical achievements; they are a testament of resilience, determination, and unwavering spirit of athletes from all corners of the world. At Careteam Technologies, we draw inspiration from the dedication, long hours of training and excellence displayed by these athletes. This pursuit of excellence motivates us to dream big, push boundaries in our work, to innovate, and empower individuals to achieve their own form of greatness. The Olympics remind us that despite our differences, we are united by a common drive to overcome challenges and achieve extraordinary things. This spirit of perseverance and triumph is at the heart of what we do at Careteam Technologies. Pierre de Coubertin (1863 - 1937) was a French educator and historian known as the father of the modern Olympic Games and he would have been no doubt so pleased to have the games take place in his country.     HOT OFF THE PRESSES: Appreciate the shout out Canadian Healthcare Technology/ Jerry Zeidenberg! (https://lnkd.in/gW9N963b). User experience has been neglected in healthcare for too long, causing frustrations, inefficiencies and burnout. Great to see investments in making digital health easy to use recognised on the global stage. It is all about humanising digital health and putting humans first! And doing that for healthcare professionals and for patients. Technology can be a force for good when it fits into real life workflows, empowers the human users and makes our lives better. One of our big inspirations was Howard Behar, and his ideas captured in his book "It's Not About the Coffee: Lessons on Putting People First from a Life at Starbucks" (https://lnkd.in/gRHPj4Ka?) that he also generously shared with me when I started building out my concepts behind Careteam. THANK YOU Howard!!! Mr. Behar was President of North America and he became the founding President of Starbucks International, opening the very first store outside of North America in Japan. During his tenure, he participated in the growth of the company from only 28 stores to over 15,000 stores spanning five continents.     Great op-ed in the #GlobeandMail from one of our great partners, Carolyn Steele Gray, Canada Research Chair in Implementing #DigitalHealth #Innovation at the Science of Care Institute and Lunenfeld-Tanenbaum Research Institute, Sinai Health Canada. Here are some key messages: ⭐ “Care co-ordination requires a collaborative approach, with patients, caregivers and clinicians all working in tandem with the managers and leaders guiding the process, and with the organizations and provincial bodies funding, regulating and governing the system”. ⭐ ”For co-ordination to really occur, information not only needs to be available, but it needs to be understandable by those who read it (including patients), while being available when it is required, and actually fitting in with how our already-overtaxed clinicians and health staff do their work". ⭐ ”There is an opportunity to use this momentum to set up partnerships, funding models, care delivery networks and engagement strategies to ensure information sharing actually leads to the proper co-ordination of care for Canadians”. We support Dr. Steele Gray’s position that while Bill C-72 is a step in the right direction, on its own, it is not sufficient to help with co-ordinating care. It helps address only one key factor of coordination: information sharing. Bill C-72, the Connected Care for Canadians Act, was introduced in June by Health Minister Mark Holland. This Act is designed to allow Canadians secure access to their health data, thereby empowering patients to make informed decisions and enhancing the quality of care they receive from healthcare providers.     The inspiring quote we chose for this edition was inspired by the recent post by Tim Murphy with another de Bono quote "You cannot look in a new direction by looking harder in the same direction.” ✨ ✨ ✨ Dr. Edward de Bono, a physician by training, pioneered innovative thinking methods that enabled transformative change in problem-solving and decision-making processes. His insights on creativity, lateral thinking and how to accomplish change have become the bedrock of innovation and creative efforts since. Author of over 60 best seller books, he is perhaps best known for his book about the "Six Thinking Hats", a novel way of investigating an issue from a variety of perspectives that can be used by individuals or groups to move outside habitual ways of thinking, try out different approaches, and then think constructively about how to move forward. One needs to remember that all of these different perspectives are useful and ensure none are forgotten in the planning and analysis of any change initiative: ⚪ White Hat: Focuses on facts, data, and information available or needed. 🔴 Red Hat: Involves emotions, feelings, intuition, and gut reactions. ⚫ Black Hat: Considers risks, potential problems, and critical judgment. 🟡 Yellow Hat: Looks at benefits, opportunities, and positive outcomes. 🟢 Green Hat: Encourages creativity, new ideas, and alternative solutions. 🔵 Blue Hat: Manages the thinking process, sets goals, and outlines agendas. #inspiring #quote #creativity #leadership #healthcare #physician

In the 40 years, they have accomplished: ♿ Over 87,000 persons with disabilities served, with Employment Programs, Computer Tutoring and Assistive Technology Programs, Ergonomic and Assistive Technology Services for Employers and so much more 🏆 One Guinness book of record set, the Makers Making Change initiative for the most people to take an online assistive technology lesson in 24 hours ⭐ Thousands of participants, staff, volunteers, partners, and supporters. The legacy of Neil Squire who at 21 years old after surviving a terrible car accident, paralysed from the neck down and could hardly speak, yet decided to use this challenge to make a difference in the world reminds of the story of another Canadian Rick Hansen and all of his achievements for people with disabilities - real life inspiring heroes to whom we collectively owe so much.     We need to better guide and support patients across the options available to them in our complex health system, and it's a thrill to see a partnership come together to make this a reality for people in the Mississauga region with the launch of the Mississauga Health Seniors Navigation Portal! It has been wonderful to work under the leadership of Partners Community Health, Trillium Health Partners and the Mississauga Health (Mississauga Ontario Health Team M-OHT), together with Caredove, Central Registry and many others, to make this come together. Thank you to the Ontario Centre of Innovation (OCI) and the National Research Council Canada / Conseil national de recherches Canada for the funding and the support too! This portal is a comprehensive digital system designed to support older adults in living well at home by helping connect them digitally with the services they need and also provide access to a care navigator for assistance, services such as meals and food security, transportation services, home support, exercise programs, day programs, respite care, and more. All of our efforts with Baycrest's Centre for Aging + Brain Health Innovation (CABHI), DIGITAL, AGE-WELL NCE Inc., AARP Innovation Labs, C2MP, StartUp Health labs led to this day!
