The most important thing in communication is hearing what isn't said” Peter Drucker. After every mission or trip—whether it’s a humanitarian mission, disaster relief operation, or wilderness expedition—conducting an after-action review (AAR) is critical. The AAR helps us reflect on what went well, identify areas for improvement, and solidify the lessons learned. It’s about ensuring continuous growth, both individually and as a team. In my experience, especially from previous combat deployments, I’ve learned that it’s equally important to address the human side of these experiences. Medical professionals often approach the AAR with a logical mindset, focusing on operational outcomes. However, we must also take the time to review our human feelings and emotional responses. Recognizing and discussing the impact of what we’ve experienced builds stronger, more resilient teams. Just as important as the AAR is the recovery phase. Returning home means reconnecting with family, decompressing from the intense experience, and ensuring that both physical and mental health are prioritized. Balancing the high-stress demands of our work with the need for personal recovery is essential for sustained performance and well-being. The work isn’t truly finished until we’ve taken the time to review, recover, and reconnect—both professionally and personally. My list for improvement: 1. Improve my Spanish. 2 Train more with OBGYN ultrasound skills. 3. I’ve also been getting into something called outcomes based care (OBC), which basically means, I am working to ensure that Head & Heart Therapy ACTUALLY helps people to change. Because, although I do believe that personal growth is a lifelong process, I also know that the research shows that therapists are NOT great at predicting outcomes and they often rate themselves better than they actually are!!! 😅 So I encourage you to ask your therapist (or prospective therapist): - How do I know this is going to be effective?  - What are my treatment goals (by law all therapists must have a diagnosis and treatment plan)? - What sets you and your clinic apart?  - What evidence based practice do you use and what training do you have? To be fair, Mike Tyson mouth punched a lot of people's plans. So why do so few people do "Advanced Care Planning"? Probably because we forget that life can punch... and because our system doesn't value early ACP, either. This type of planning is about relationship. A relationship between person and system. If it isn't valued by either party or by only one party, it has no value. It has to be valued by both. If not, the dominant culture, dominates. That plan "wins". If both parties value a planning relationship it's value increases proportionately to their investment. And that relationship can evolve with time. In our acute care system, if a person living with advancing frailty is worn down after waiting, not asked, doesn't have access to the good washrooms or a decent cuppa, is both sick and exhausted... and then gets plugged into the home advantage of algorithmic hospital medicine, feels psychologically dominated, scared, unsafe and thus less likely to speak or be heard; they and their ACP gets a system delivered mouth punch every time. It takes leadership to see, feel, and plan for everyone's benefit. If healthcare leaders don't value ACP in our culture, we waste our time and the hurt continues. It's a curious culture that supports ACP and is a different way of caring. A relationship. No independent patient effort can effectively and consistently challenge our medical culture. We are the dominant force. Often, like a boxer an ACP isn't written anywhere, until it is. And yet it is in action all the time. It has dynamic elements. For better or worse. It is in asking and listening. An investing together in something that WE support. Valuing, planning, updating should be a priority that is pared with traditional medical skills and strong system leadership. Understanding in the value and care currency of WMTY, ACP relationships start at the top. It is a powerful and beautiful opportunity to invest in each others wellbeing. It is time for us to make it safe to share. To help each other value and hold each others plan. Regardless of how hard life hits us.
