Today, we had the honor of recognising outstanding contributions to Patients’ Education and healthcare at Centre for Health Activation (CHA) monthly hospital conference. The event featured inspiring insights from our award winners and presenters, all focused on enhancing patient experiences and fostering collaboration. Key Highlights: 🔹 Dr. Ee Yuee Chan emphasised the importance of co-creating healthcare solutions with caregivers, reminding us to continually ask, “What matters to you?” in our interactions. 🔹 Dr. Clive Tan Tan shared a powerful perspective on seeing patients as individuals with stories, not just a list of medical issues. This human-centered approach is vital for personalised care. 🔹 Dr. Chiam Zi Yan highlighted the collaborative efforts in the Breathlessness Ecosystem, showcasing how partnerships across disciplines can empower patients with chronic breathlessness through education and support. 🔹 Ms. Sylvie Chua discussed the transformative journey of patient advocacy, encouraging us all to partner with patients in developing meaningful education resources. “Never doubt that a small group of thoughtful, committed, citizens can change the world. Indeed, it is the only thing that ever has.” Paving the way for patient advocacy with our patient support groups, one step at a time. The Centre for Health Activation celebrated Health Literacy Month, in partnership with our passionate patient support group volunteers and healthcare professionals in Tan Tock Seng Hospital. After months of co-creation and ideation with TTSH Colorectal Patient Support Group members, we launched a dedicated Patient Education space at Clinic 2A. We are grateful for the strong support of Tan Tock Seng Hospital’s clinical and operations team for jointly embarking on this journey of patient advocacy. By committing a dedicated space within TTSH Clinic 2A to support our patients and caregivers, it demonstrates the strong commitment of CHA and NHG institutions in charging up the community with the knowledge, skills and confidence to self-care and to navigate the health system. We invited Mr Prabu Naidu, Leader of TTSH ColRect Patient Support Group, to share his story of cocreation with our healthcare community this morning through TTSH Hospital Conference, attended by more than 300 healthcare professionals. As a cancer patient himself, he fully understands the emotional roller coaster journey and the challenges in navigating health resources. Having visible and dedicated spaces within TTSH such as the PE booth and PE gallery allow our patient support group members to provide befriending and psychosocial support for fellow patients and to help navigate the comprehensive suite of patient education resources developed by our healthcare professionals. Our patients and caregivers are never alone in the healing journey as we believe in building and growing caring communities, in partnership with our health and social care partners. It takes a village to build better health. Everyone has a role to play in caring for themselves and their communities and it’s a journey we are committed to building a healthier future together. The CHI Fellowship participants had the privilege of attending a discussion featuring Professor Jonathon Gray, who’s a leading expert in innovation and leadership, about the importance of systems thinking, storytelling, and experiential learning for driving change. With his trademark humility, honesty and generosity, Professor Gray took the class through these key insights: 1️⃣ Innovation in Crisis: Prof. Gray shared an inspiring example from the COVID-19 pandemic, where he led the transformation of a stadium into a 400-bed hospital in just a month, collaborating with military support. This was a reminder that change can be managed effectively, even in the most challenging times. 2️⃣ The Power of Connection: He emphasized that success often lies in our networks and connections. As leaders, we can’t just instruct; we need to inspire and engage others to innovate collaboratively. 3️⃣ Storytelling as a Tool: In complex situations, the ability to tell a compelling story is crucial. It helps us connect with others and navigate challenges more effectively. 4️⃣ Learning from Chaos: Prof. Gray highlighted the importance of understanding chaos and knowing who to turn to for guidance.
