In honor of Dr. Martin Luther King Jr.'s enduring legacy, Santa Clara Family Health Plan (SCFHP) was proud to support the African American Community Services Agency's 45th Annual MLK Luncheon. Dr. Ria Paul MD, Chief Medical Officer, Teresa Chapman, Chief People Officer, and Pamela Lowe, Director of Vendor Management, represented SCFHP at the event on January 20 at San Jose State University's Student Union Ballroom. The MLK Luncheon reminds us of Dr. King's powerful legacy and the importance of advancing equity and justice in our communities. At SCFHP, we believe that by working together with our community partners, we can create meaningful change and improve the health and wellness of our African American members. Santa Clara Family Health Plan (SCFHP) is a local, community-based health plan dedicated to improving the health and well-being of the residents of Santa Clara County. This past June, Santa Clara Family Health Plan (SCFHP) launched its Community Health Worker (CHW) Certification Training Program. On December 7, SCFHP proudly celebrated the graduation of 29 newly certified CHWs, with an outstanding 85% graduation rate. The goal of SCFHP’s CHW Certification Program is to create a clear pathway for CHWs to become certified, learn and practice important skills, and meet the California Department of Health Care Services (DHCS) requirements to bill for services provided under the CHW Medi-Cal benefit. This Program is made possible because of the generous support of SCFHP’s Board Innovation Fund, which demonstrates SCFHP’s commitment to advancing the CHW workforce and investing in the health of our community. One of our graduates shared, “We have already been doing a lot of the stuff we are learning, but we get to practice it with role –playing so we can be better.” Another graduate noted, “One of the biggest takeaways is the networking with other peers in the class—it’s been the best way to learn more about resources in Santa Clara County.” A heartfelt thank you to SCFHP’s curriculum developers and training partners at Foundations Group Consulting and our training administrator, Catholic Charities of Santa Clara County, for their invaluable collaboration in the program. Congratulations to all the graduates! Your dedication to improving the health and well-being of our community is inspiring!

Santa Clara County is one of the first counties in the State of California to implement the Justice-Involved Reentry Initiative. Upon release from incarceration, individuals can now opt into continued care management services, and can qualify for Enhanced Care Management (ECM), a Medi-Cal benefit. Our Community Based Services Specialist Daljit T. is part of the SCFHP team supporting the Justice-Involved Reentry Initiative. Daljit says, “Launching the Justice-Involved initiative takes building bridges between pre- and post- incarceration services, and through dedicated internal efforts and collaboration with Anthem Blue Cross, Kaiser Permanente, and County of Santa Clara’s Office of System Integration and Transformation, SCFHP is making significant progress in meeting the requirements and prioritizing the needs of this population." SCFHP is responsible for coordinating community based organizations that are providing ECM services and high quality care management to our members starting at release from incarceration. Those individuals who opt into pre-services (up to 90 days prior to their release), may receive services such as: - Physical and behavioral health clinical consultation - Lab and radiology services - Medication-assisted treatment (MAT), medications and medication administration - Community health worker (CHW) services - Provision of medications and durable medical equipment (DME) To learn more visit: https://lnkd.in/ge9vRR-k

Santa Clara Family Health Plan extend our sincere appreciation to the many providers and their dedicated teams who attended our annual Provider Meet and Greet, hosted by our Provider Network Operations Department on October 16, 2024. Providers were warmly welcomed by Chief Executive Officer Christine Tomcala and Chief Medical Officer Dr. Ria Paul, M.D., who engaged with attendees throughout the afternoon. The event brought together 85 representatives from various provider organizations, fostering a valuable opportunity for networking and collaboration. Representatives had the opportunity to speak with each other, and learn about the impact everyone’s work has on SCFHP members. Some of our contracted vendors attended such as QUALITY HEALTH PARTNERS and Protoqual, providing valuable information on their services. Finally, there was an added bonus to the afternoon - attendees enjoyed complimentary lunch, branded swag, and raffle goodies! “We relish a wonderfully collaborative relationship with SCFHP,” said Ivan Gonzalez, Assistant Manager at Yellow Checker Cab Company, Inc. of Silicon Valley, a transportation provider for SCFHP. “It has been integral to our shared success. Over the years, regulatory and compliance requirements have changed. And, we’ve worked hand in hand to ensure the transportation model has evolved to comply with those changes.
