About us

At Plan Partners, we are committed to helping our customers bring their NDIS plans to life by offering two services: Plan Management and Support Coordination. Our unique approach to the disability sector makes a real difference in people’s lives every day as we help them navigate the complexities of the system and unlock the potential of their NDIS plans. Plan Management makes it easier for our customers to manage their NDIS funds by freeing them from the time-consuming daily administration tasks. Our Support Coordinators work closely with our customers to understand their needs and goals, and then help find and connect them with people who are a good fit for their situation. We always put our customers first. We are a committed partner, as displayed in our Customer Charter, supporting them at every stage of their NDIS journey.

    In our 'meet the team' series, we're introducing our Support Coordinators who are here, as their title suggests, to support NDIS participants. It's time to meet Stacy, one of team in Victoria. Stacy speaks French and Mauritian Creole and her expertise is supporting participants who are Aged Care residents under the age of 65.   Originally a Plan Manager, Stacy moved into Support Coordination. She has an intimate knowledge of the NDIS and has a child with disability who is supported by the Scheme.   Stacy: “I am very good at knowing what information to gather for reporting periods. Having my own child who is on the NDIS gives me the passion and drive to succeed.     We are VERY excited to announce Plan Partners has been named as a finalist in the "Most Outstanding Plan Management Organisation" category of the 2024 Australian Disability Service Awards. This is testament to the exceptional work our team deliver every day to our customers and their providers. We are looking forward to the awards evening in November where the winner will be announced.     It's Brain Injury Awareness Week and we're spotlighting one of our participants, Mark, who lives with an acquired brain injury. Find out more here, including some great links if you or someone you know needs support. Name: Mark Korenhof, I am 58 and live in Cranbourne West in supported accommodation. My family lives nearby. How long have you lived with an acquired brain injury? I have had a brain injury for the last 24 years. I had a large pituitary brain tumour. How does it affect your day-to-day life? It kills all my motivation, I was active before and now I'm not very active at all. My memory is slower and I have some trouble with my vision. What do you wish people knew about what it’s like to have an ABI? Don't judge a book by its cover because not all disabilities are obvious like being in a wheelchair. What have you found helpful? Going to the Brain Injury Matters peer support group and getting on the NDIS. I am lucky to get so much support.
